Zusammenfassung
Die Komplikationen bei Brustkorbverletzungen sind auf zum Teil schwer oder spät nachweisbare intrathorakale Organbeteiligungen zurückzuführen. Das Fehlen äußerer Verletzungszeichen ist dabei kein Beweis für die Unversehrtheit der Thoraxorgane. Durch die intrathorakalen Verletzungen können Veränderungen der Druckverhältnisse in der Brusthöhle eintreten, und über eine Störung des kardio-respiratorischen Systems kann ein katastrophaler Kreislaufzusammenbruch bewirkt werden. Die häufigste Komplikation stumpfer und penetrierender Thoraxverletzungen ist der Hämopneumothorax.
Stumpfe und penetrierende Verletzungen des Herzens und der großen Gefäße bedürfen einer schnellen gezielten Diagnostik und möglichst frühzeitiger operativer Revision, die auch bei Verletzungen von Trachea, Bronchien und Ösophagus angestrebt werden muß. Die Letalität der Kontusionslunge liegt heute noch bei 50 bis 90% und stellt weiterhin eine therapeutische Herausforderung dar.
Bei Verdacht auf Zweihöhlenverletzungen sollte bei primärer Thoraxeröffnung über eine Zwerchfellinzision der Bauchraum inspiziert werden; in 75% muß dieser jedoch zuerst revidiert werden.
Abstract
Complications after chest-injuries are due to lesions of intrathoracic organs. Sometimes it is difficult to recognize these lesions, therefore the diagnosis might be delayed. Even if there are no visible signs of an injury, intrathoracic organs might be damaged considerably. Due to lung-injuries the pressure inside the thorax might change; disturbances of the cardiorespiratory system may result in a complete breakdown of the circulation. The most common complication of closed and open chest-injuries is a hematothorax. Perforating and closed injuries to the heart and the great vessels require rapid diagnosis and surgery as early as possible. The same applies to lesions of the trachea, bronchialtree and esophagus. The mortality of a lung-contusion is even today between 50–90%, it is a real therapeutic challenge.
In 75% of cases of combined injury to the thoracic and the abdominal cavity, the abdomen has to be operated first. If a thoracotomy has been done in first place and if an intraabdominal lesion is suspected, an inspection of the abdomen should be done through an incision of the diaphragm.
