Alloarthroplastik, Resektionsarthroplastik und Arthrodese des Schultergelenkes — Indikation und Technik

Zusammenfassung

In der vorliegenden Arbeit wird über die an der BG-Unfallklinik Duisburg-Buchholz übliche Indikation und Operationstechnik bei Problemsituationen im Schulterbereich berichtet. In Frage kommende Operationsmaßnahmen sind Alloarthroplastik, Resektionsarthroplastik und Arthrodese. Die Indikation zu den einzelnen Operationsverfahren wird dargestellt, wobei insbesondere herausgestellt wird, daß aufgrund noch nicht gelöster technischer Probleme die Alloarthroplastik im Schultergelenksbereich nur eine eng begrenzte Indikation besitzt und ausschließlich älteren Patienten vorbehalten bleiben sollte. Anschließend werden Details der Operationstechnik der verschiedenen Operationsmethoden beschrieben. Abgesehen von der einfachen Resektion des Oberarmkopfes handelt essich dabei um technisch schwierige unfallchirurgisch-orthopädische Spezialoperationen, welche einen erfahrenen Operateur erfordern.

Abstract

Indications and operative technics of problematic situations in the shoulder region are discussed. Methods which are commonly used at the “BG”-Accident-Hospital, Duisburg, are described in detail. Operative methods under question are arthroplasty, resection-arthroplasty and arthrodesis. The indications for the individual operative procedures are presented. Particular emphasis is laid upon the fact that alloarthroplasty — because of still unsolved technical problems — retains only a very narrow indication. It should be limited exclusively to older patients. Details of the operative technic for various operations are decribed. Apart form the common resection of the humoral head these are technically difficult; they require a surgeon who is experienced in traumatology and orthopedics.
