Frontobasale Frakturen im Wachstumsalter

Zusammenfassung

Anhand von 7 Patienten im Alter von 2 bis 14 Jahren wird auf die Komplikationsbereitschaft frontobasaler Frakturen, vornehmlich im Kindes- und Jugendalter hingewiesen. Die wesentlichen Schlußfolgerungen: 1. Die Chance auf eine dauerhafte Spontanheilung der traumatisch bedingten Kommunikation ist trotz der ausgezeichneten Reparationsfähigkeit bei Kindern und Jugendlichen gleich schlecht wie bei Erwachsenen. 2. Eine indirekt offene frontobasale Fraktur ist aufgrund des Entwicklungsablaufes der Nasennebenhöhlen ab dem 1. Lebensjahr möglich. Als weitere Konsequenz wird für eine weitgestellte Operationsindikation wie folgt eingetreten:

• 1. Alle direkt offenen Frakturen,

• 2. alle indirekt offenen Frakturen; a. nachgewiesene Kommunikation b. verschobene Fraktur c. entzündliche Komplikation (Meningitis) d. präexistente Entzündung der Nasennebenhöhlen.

Abstract

On the basis of own experiences in seven cases of frontobasal fractures in patients between 2 and 14 years, the tendency for complications especially in this age group is pointed out. The main conclusions are: 1. In spite of an excellent tendency for healing in children, the chance of a spontaneous closure of the traumatic communication is as bad as in adults. 2. With the termination of the development the possibility of an indirect compound fracture is given after the first year of life. The consequence of the above mentioned facts is, that there exist wide indications for surgical repair: 1. all direct compound fractures, 2. indirect compound fractures with a. proved communication, b. dislocated fractures, c. meningitis, d. preexisting inflammation of the sinus. Moreover we prefer in all cases the neurosurgical, bifrontal approach.
