Zusammenfassung
Im Zeitraum von drei Jahren wurden 33 freie Latissimus-dorsi-Lappenplastiken und 23 gestielte Myoplastiken zur Defektdeckung bei ausgedehnten Weichteildefekten und Knocheninfektionen ausgeführt. Die Technik des freien Muskeltransfers mit mikrovaskulärem Anschluß wird detailliert dargestellt und die Behandlung in der frühen postoperativen Phase erläutert. Es wird über die Ergebnisse der freien und gestielten Muskelplastiken berichtet. Die Erfolgsquote bei freiem Lappentransfer beträgt im eigenen Krankengut 88%, bei gestielten Myoplastiken 65%. Nach Sanierung des Weichteildefektes mittels myoplastischem Verfahren scheint die Frakturheilung bzw. Infektsanierung am Knochen günstig beeinflußt zu werden. Vor-und Nachteile freier und gestielter Lappen werden diskutiert und abschließend die Indikationen für beide Verfahren genannt.
Abstract
Over the period of three years 33 free latissimus dorsi-flaps and 23 pedicled myoplasties of various typs for the coverage of large tissue and bone defects are performed. The operative technique of the free muscle transfer with microvascular anastomosis are described in detail. The postoperative management is outlined. The results of the free and pedicled muscle flaps are reported. In free transplantations the sucess-rate was 88% and in pedicled flaps 65%. There is good evidence that bone consolidation and healing of the chronic bone infection can be improved by healthy vital soft tissue coverage. The indications for both procedures and the advantages and disadvantages of free and pedicled flaps are discussed.
