Management des Polytraumas—Möglichkeiten und Grenzen im Krankenhaus der Grund-und regelversorgung

Zusammenfassung

Die Möglichkeiten und Grenzen, die sich im Krankenhaus der Grund-und Regelversorgung bei der Behandlung polytraumatisierter Patienten ergeben, werden am Beispiel des Marienkrankenhauses Trier-Ehrang erörtert. Bestimmte räumliche, apparative, personelle und organisatorische Voraussetzungen müssen vorhanden sein. Entscheidende Bedeutung kommt der Kooperation zwischen Anästhesist und Chirurg zu. Vorteilhaft sind kurze Wege, kurze Zeiten und ein eingespieltes Team. Nachteilig sind fehlende diagnostische Möglichkeiten, wie Computertomographie und Angiographie. Schädel-Hirn-Verletzungen und Frakturen der Wirbelsäule müssen in ein entsprechendes Zentrum verlegt werden.

Abstract

The chances and the limits in the treatment of patients with multiple trauma are discussed on the example of the Marienkrankenhaus Trier-Ehrang. Definite conditions concerning to the rooms, the staff and the organisation must be realized. Most important is the cooperation between the anaesthesiologist and the surgeon. Short distances, short times and a well trained team are advantageous. If computertomography and angiography are not available the diagnostical facilities are limited. Severe craniotrauma and spine fractures must be transfered to a special medical centre.
