Zusammenfassung
Von 42 revaskularisierenden Eingriffen bei der Hüftkopfnekrose des Erwachsenen im röntgenologischen Stadium II und III (Ficat 1980) wurde in fünf Fällen ein durch den Ramus profundus der A. glutea sup. versorgter kortikospongiöser Beckenspan in den Hüftkopf transplantiert. In den andern Fällen war der durch die A. circumflexa ilium prof. perfundierte Beckenspan gewählt worden. Beide gefäßgestielte Beckenspäne erlauben es, Spongiosa mit erheblicher osteoinduktiver Potenz in den Hüftkopf zu verlagern. Dargestellt werden die operationstechnischen Vorteile des von uns inaugurierten gefäßgestielten, durch den Ramus profundus der A. glutea sup. perfundierten (lateralen) Spanes im Gegensatz zum medialen gefäßgestielten Beckenspan (A. circumflexa ilium prof.). Die postoperative klinische Nachkontrolle der operierten Hüftgelenke läßt gewisse Vorteile des lateralen gefäßgestielte Beckenspanes gegenüber dem medialen erkennen. Dieses dokumentiert sich besonders in den postoperativen Bewegungsausmaßen der transplantatversorgten Hüftgelenke. Die in allen Fällen vorgenommenen superselektiven Gefäßstielangiogramme und Transplantatlagervenogramme werden vorgestellt und diskutiert.
Abstract
The development and introduction of microsurgical technique has made possible completely new concept of revascularisation surgery in femoral head necrosis. The bone graft described in this presentation is a pedicled one from the lateral ilium. In five femoral head necrosis a pedicled corticocancellous pelvic graft, supplied by the deep branch of the superior gluteal artery, was transplanted in the femoral head. In all cases it was possible to lift out the pelvic graft from the same side as the affected hip joint. Contrary to the medial iliac bone graft (deep circumflex iliac artery) the lateral iliac bone graft (deep branch of the superior gluteal artery) is inserted into the femoral head dorsally. Preoperatively and postoperatively performed selective angiographies are necessary and presented. In all cases an unimpeded perfusion could be shown three months postoperatively. Advantages and indications of the pedicled bone graft are discussed.
