Abstract
The aim of this investigation has been to test in humans the reliability and validity of three painful stimuli, i.e, mechanical pressure, a modified submaximum-effort tourniquet technique, and the cold pressure test. Twenty-four untrained subjects (twelve females, twelve males) volunteered in the experiments and, after a trial session were presented with four runs of these three noxious stimuli within 36 h. They assessed the painfulness using a direct scaling technique (category subdividing procedure). It was shown that all pain-induction methods gave reliable results (Cronbach's α>-0.90). Significant intercorrelations between the results of the application of mechanical pressure and ischemia support the validity of these two stimuli. Stimulation by cold, on the other hand, did not reveal such coherence. Repeated mechanical stimulation caused sensitization that did not occur with ischemia or cold. There were no significant differences whether the experiments were performed in the morning or in the evening. Experimental pain induction either by mechanical pressure or by ischemia judged in accordance with the category subdividing procedure produce reliable and valid results; the methods are easily applicable and economic.
Zusammenfassung
Es wurde untersucht, ob ein mechanisches Druckreizverfahren, eine modifizierte “submaximum effort tourniquet technique” und Schmerzstimulation durch Kälte im algesimetrischen Humanversuch sinnvoll und routinemäßig eingesetzt werden können. Bewertungskriterien dazu waren die experimentell zu ermittelnden Gütekriterien. An 24 freiwilligen, untrainierten Versuchspersonen (zwölf Frauen, zwölf Männer) wurden, nach einer Probesitzung, vier Serien von drei Reizverfahren in einem Zeitraum von 36 Stunden appliziert. Die induzierte Schmerzintensität wurde mit dem Kategorienunterteilungsverfahren erfaßt. Die drei Schmerzreizmethoden wurden problemlos von den Probanden toleriert. Für alle drei Verfahren konnten sehr hohe Zuverlässigkeitskoeffizienten ermittelt werden (Cronbach's α>-0.9). Signifikante Interkorrelationen wurden zwischen den Druck- und Ischämiereizverfahren gefunden, wodurch deren Validität gestützt wird. Zwischen Kältereizung und den anderen Verfahren zeigten sich keine signifikanten Zusammenhänge. Wiederholte Reizungen sensibilisierten die Probanden für Druckschmerz, nicht jedoch für Kälte- und Ischämie-schmerz. Die Ergebnisse der Messungen am Morgen und am Abend unterscheiden sich bei allen drei Verfahren nicht bedeutsam. Die Versuche zeigen insgesamt, daß die Schmerzinduktion durch mechanischen Druck und durch Ischämie bei Messung der induzierten Schmerzintensität mit dem Kategorienunterteilungsverfahren praktikable, ökonomische, reliable und valide Verfahren sind.
