Operationsprinzip
Durchtrennung des ligamentären Tunneldaches, unabhängig von der Ursache der Kompression, als wichtigste Maßnahme, allein oder gegebenenfalls kombiniert mit zusätzlichen Eingriffen, wie Synovektomie der Beuger, peri- oder endoneurale Neurolyse. Nur bei Verkleinerung der Tunnelkapazität und bei raumverdrängenden Prozessen in Tunnelnähe ist eine Spaltung und anschließendes Offenlassen des Retinaculum flexorum, nach Entfernen der Ursache (Reposition des luxierten Os lunatum, Exstirpation eines Ganglion), ausreichend. Der Vorschlag zur operativen Behandlung durch Spaltung des Retinaculum flexorum stammt von Marie und Foix [7]. Die Operation wurde erstmals von Learmonth 1933 durchgeführt [5].
