Die ventrale interkorporelle Spondylodese der Halswirbelsäule

Operationsprinzip

Nach exakter anatomischer Reposition eines verletzten Halswirbelsäulensegmentes wird über einen ventralen Zugang der Intervertebralraum ausgeräumt und mit einem autogenen kortikospongiösen Block vom Beckenkamm überbrückt. Die interkorporelle Spondylodese wird mit einer H-Platten-Osteosynthese abgeschlossen.

Anzustreben ist immer eine Minimalfusion. Bei diskoligamentären Verletzungen ist eine Einsegmentfusion ausreichend; bei Trümmerfrakturen sind eine partielle oder totale Spondylektomie mit oder ohne Resektion der Hinterwand und eine Zweisegmentfusion notwendig.
