Summary

The described system not only allows cementing under vacuum, it also permits to optimise drainage during surgery. At the same time it allows to centre the stem. The system consists of a specially designed cannulated stem of the femoral component and a special intramedullary plug made from PMMA to which a rigid suction cannula is attached. Once introduced the plug will be connected to a suction pump via the suction cannula. The cement will then be introduced with a gun. There-after the centrally cannulated stem will be inserted over the suction cannula. After hardening of the cement will the suction cannula be removed.

The system has been used in 102 patients since September 1994 (until November 1995).

In all but one the tip of the stem was well centred. The axial alignment of the stem in the AP plane was judged correct in 88 patients who had also an intact cement mantle. In the lateral view the alignment was correct in 99 instances. An intraoperative fat embolism was never observed. Postoperatively a lung embolism occurred twice and twice a transient respiratory distress syndrome.
