Abstract
During isolated limb perfusion (ILP) severe metabolic impairment with a subsequent alteration in oxygen consumption can be observed. The mechanisms responsible for this may be extracorporeal circulation, hyperthermia, and appliation of cytostatic drugs and cytokines. Thirty-three patients underwent ILP with rhTNFα and melphalan for melanoma or soft-tissue sarcoma. Cardiopulmonary monotoring consisted of arterial and mixed venous blood-gas analysis and a Swan-Ganz catheter was inserted after induction of general anesthesia prior to any surgical intervention. Arterial (SaO2) and mixed venous (SvO2) oxygen saturation, serum lactate and end-expiratory CO2 concentration were determined peri-and postoperatively for 72 h. Oxygen supply and consumption rates were measured systemically (DO2I, VO2I) and in the extracorporeal circuit (‘DO2I, ‘VO2I). For statistical analysis we used thet-test. During extracorporal circulation an increase of DO2I and VO2I was observed. A slight increase of lactate values began during the wash-out phase. Immediately after reperfusion, DO2I, VO2I and lactate increased significantly with normalization until the 2nd postoperative day. SaO2 and SvO2 remained unchanged. A significant correlation between regional toxicity and the postoperative maximum of serum lactate values was found. The increase of DO2I and VO2I in the tissues during ILP and after reperfusion was achieved by a significant increase in cardiac output while the oxygen extraction rate was not altered. Elevation of lactate values after reperfusion and the increase in oxygen utilization might be due to oxygen depletion in the perfused limb. This could contribute to the development of lactacidosis or rhabdomyolysis. There-fore, to minimize toxicity it seems to be mandatory to measure adequate tissue oxygen supply during ILP.
Zusammenfassung
Im Rahmen einer isolierten Extremitätenperfusion (ILP) können durch extrakorporale Zirkulation, Hyperthermie und Applikation von Zytostatika und/oder Zytokinen metabolische Veränderungen mit konsekutiv alteriertem O2-Bedarf induziert werden. Bei 33 Patienten wurde eine ILP wegen eines Melanoms oder Weichgewebesarkoms mit rhTNFα und Melphalan vorgenommen. Über einen präoperativ eingeschwemmten Swan-Ganz-Katheter wurden die kardiozirkulatorische Funktion überwacht sowie arterielle und gemischtvenöse Blutgas-analysen vorgenommen. Arterielle (SaO2) und gemischtvenöse O2-Sättigung (SvO2), Serumlaktat und endexpiratorische CO2-Konzentration (ETCO2) wurden intra- und postoperativ bestimmt. O2-Versorgung und O2-Verbrauch wurden für die systemische Zirkulation (DO2I, VO2I) und den Perfusionskreislauf (‘DO2I, ‘VO2I) nach der Körper-oberfläche berechnet. Die statistische Auswertung erfolgte mit dem t-Student-Test. Während der extrakorporalen Zirkulation kam es zu einem Anstieg von DO2I und VO2I, in der Auswaschphase begann ein Anstieg des Serumlaktats. Unmittelbar nach Reperfusion trat ein weiterer signifikanter Anstieg von DO2I, VO2I und Laktat auf, der sich im Verlauf der ersten beiden postoperativen Tage normalisierte. SaO2 und SvO2 blieben unverändert. Die regionale Toxizität korrelierte mit dem maximalen postoperativen Laktatanstieg. Die gesteigerte DO2I und VO2I der Gewebe während Perfusion und nach Reperfusion wurden durch eine signifikante Erhöhung des Herzzeitvolumens erreicht. Eine Erhöhung der O2-Ausschöpfung wurde nicht beobachtet. Der Laktatanstieg nach Reperfusion und die Steigerung der O2-Utilisation deuten auf eine O2-Schuld der perfundierten Extremität hin, die zur Laktatazidose führt, aber auch vermittelnder Mechanismus einer Rhabdomyolyse sein kann. Die Messung der adäquaten Gewebeoxy-genierung während der Perfusion und ein Ausgleich einer Sauerstoffschuld nach Reperfusion sind zur Minimierung der Toxizität anzustreben.
