A biodegradable expansion plug for fixation of the coracoid bone block in the Bristow-Latarjet operation

Summary

In a prospective study, 33 patients with recurrent anterior dislocation of the shoulder were treated by a modified Bristow-Latarjet operation using a biodegradable poly-L-lactide expansion plug to fix the transferred coracoid bone block. The mean follow up was 12 months. No redislocations occurred. To assess incorporation of the bone block, serial CT scans were obtained in 18 randomly selected patients and 15 showed solid fusion. The shape of the implant was not affected by degradation during the first 18 months and there were no signs of inflammatory foreign body reaction. This type of plug avoids the complications which have necessitated the removal of metal devices and is a promising new method of fixing the transferred coracoid.

Résumé

Dans une étude prospective, 33 patients atteints de luxation antérieure récidivante de l'épaule ont été traités par un procédé de Bristow-Latarjet modifié, en utilisant un clou expansible de poly-L-lactide pour la fixation du bloc osseux coracoïdien transféré sur le col de l'omoplate. Trentetrois patients ont pu être suivis régulièrement pendant au moins 6 mois. Le temps d'observation a été de 6 à 28 mois. Chez 4 patients les radiographies de contrôle ont révélé une absence de fusion du bloc osseux coracoïdien. Il ne s'est pas produit de reluxations. Des examens tomo-densitométriques ont été réalisés chez 18 patients, dont 15 ont montré une solide fusion entre la coracoïde transférée et le col de l'omoplate. Le déficit final moyen de rotation externe a été de 8o. Il n'y a pas eu de signes de réaction à corps étranger. Pour éviter les complications qui peuvent nécessiter l'ablation des implants métalliques utilisés dans le procédé de Bristow-Latarjet, le clou biodégradable peut être considéré comme une nouvelle méthode prometteuse pour fixer le bloc osseux coracoïdien.
