Early and late displacement of fractures of the distal radius

The prediction of instability

Summary

One hundred fractures of the distal radius with dorsal displacement were treated by closed reduction and a plaster cast. The mean age of the patients was 55 years. Radiographs were taken after 1,2 and 5 weeks to evaluate the frequency of early and late displacement. Dorsal angulation occurred in 71 patients, shortening of the radius in 47 and flattening of the radial angle in 32. Late displacement was more frequent than early. Statistical analysis showed a greater incidence of secondary shortening in Older's types III and IV fractures. The severity of the initial radial shortening was the most reliable indication of instability.

Résumé

Cent fractures de l'extrémité inférieure du radius avec déplacement postérieur ont été traitées par réduction à foyer fermé et immobilisation plâtrée. Des radiographies ont été faites après 1,2 et 5 semaines afin d'estimer la fréquence des déplacements, précoces et tardifs. 71 patients présentèrent une bascule postérieure, 47 un raccourcissement du radius et 32 une diminution de l'obliquité radiale. Les déplacements ont été observés pendant toute la durée du traitement mais les tardifs ont été plus fréquents que les précoces. L'analyse statistique a montré une plus grande fréquence des raccourcissements secondaires et des angulations postérieures dans les fractures de type III et IV de Older. L'importance du raccourcissement radial initial a été reconnue comme le signe pronostique le plus fiable d'instabilité.
