Summary
Rabbit corneas were infected with Herpes simplex virus and then examined by slit lamp at intervals from three hours up to 25 days. At these times animals were sacrificed for histopathologic study of the cornea. In order to obtain plastic images of the changes at the stromal surface, the replica method of Wolf was used. The McGovern silver stain was used to study the ground substances of the corneal stroma, which is stained a deep brown, in the absence of an epithelial cover. The spaces occupied by keratocytes remain unstained. Jabonero's silver stain was used to examine the cytology of keratocytes, inflammatory cells, blood vessels, Schwann cells and nerve fibers.
Within the first two days, the keratocytes and stromal nerve fibers in the vicinity of the epithelial lesion undergo a rapid degeneration. Infiltration by macrophages and mononuclear cells is well established by the end of the first week. At the same time, blood vessels appear between stromal lamellae as long solid sprouts, that subsequently develop a lumen. Although stromal nerves underlying the epithelial lesion become fragmented, the Schwann cells remain quite resistant. The stromal surface, and in particular, the basement membrane with its complement of desmosomes, becomes disrupted. Both the loss of a normal basement membrane, as well as damage to corneal nerves can explain the failure of the epithelium to re-cover the stroma in many cases of chronic Herpes simplex keratitis.
Zusammenfassung
Kaninchenhornhäute wurden nach Infizierung mit dem Herpes simplex Virus an der Spaltlampe in Abständen von drei Stunden bis zu 25 Tagen untersucht. Anschließend wurden die Tiere zur histopathologischen Untersuchung der Cornea getötet. Für plastische Bilder der Veränderungen der Stroma-Oberfläche wurde die Replica-Methode nach Wolf angewandt. Zur Darstellung der Grundsubstanz des Hornhautstromas ohne epitheliale Bedeckung wurde die McGovernsilberfärbung benutzt. Die Grundsubstanz ist dunkelbraun gefärbt. Die von Keratozyten ausgefüllten Zwischenräume blieben ungefärbt. Zur Untersuchung der Zytologie der Keratozyten, der entzündlichen Zellen, der Blutgefäße, der Schwannschen Zellen sowie der Nervenfasern wurde die Silberfärbung nach Jabonero gebraucht.
Während der ersten zwei Tage degenerieren die Keratozyten und stromalen Nervenfasern in der Nähe der epithelialen Verletzung schnell. Am Ende der ersten Woche infiltrieren Makrophagen und mononukleäre Zellen. Gleichzeitig erscheinen Blutgefässe zwischen den Stroma-Lamellen als lange feste Keime, die dann ein Lumen entwickeln. Obwohl die unter der epithelialen Verletzung liegenden Stroma-Nerven zertümmert werden, bleiben die Schwannschen Zellen durchaus widerstandsfähig. Die Stroma-Oberfläche und besonders die Basalmembran mit ihren Desmosomen wird unterbrochen Der Verlust einer normalen Basalmembran kann neben der Verletzung der Hornhautnerven das Versagen des Epithels, das Stroma wieder zu überdecken, in vielen Fällen chronischer Herpes simplex Keratitis erklären.
Résumé
Des cornées de lapin ont été infectées par du virus herpétique et observées par la suite à la lampe après 3 heures jusqu'au 25ième jour. Le moment venu les animaux ont été sacrifiés et les cornées ont été soumises à l'examen histopathologique. Pour obtenir des images plastiques on a utilisé la méthode des répliques d'après Wolf. La substance fondamentale du stroma a été soumise à l'argentation selon Mc. Govern; elle se colore ainsi en brun foncé, alors que les espaces contenant des kératocytes restent incolores. Pour l'étude des kératocytes, des cellules inflammatoires, des vaisseaux sanguins, des cellules de Schwann et des fibres nerveuses on a eu recours à l'argentation de Jabonero.
Au cours des deux premiers jours les kératocytes et les fibres nerveuses du stroma se trouvant au voisinage de la lésion épithéliale, dégénèrent rapidement et à la fin de la première semaine l'infiltration par des macrophages et des mononucléaires est très nette. En même temps on voit aparaître entre les lamelles du stroma des vaisseaux sanguins sous la forme de longs bourgeons solides dans lesquels une lumière se creuse, ultérieurement. Malgré la fragmentation des fibres nerveuses du stroma, voisines de la lésion épithéliale, les cellules de Schwann résistent parfaitement. La surface du stroma et en particulier la membrane basale avec ses desmosomes se rupture.
Cette altération de la membrane basale associée à la dégénérescence des nerfs cornéens peut expliquer le fait que dans beaucoup de cas de kéraite herpétique l'épithélium reste incapable de recouvrir le stroma.
