Summary
In occlusive arteriosclerotic vascular disease there is no satisfactory clinical method for determining the level of amputation. Though blood circulation in skin and subcutaneous tissue can be judged fairly well, it is not as easy to evaluate muscle blood supply. Apart from arteriography in some cases, objective methods offer no help in solving this problem.
Measuring hydrogen ion concentration in the muscle however makes it possible to recognize a pregangrenous state, as it will demonstrate the increase in lactate production due to anaerobic metabolism.
For pH measurement in the tissue we used a glass electrode—protected by a metal cannula—which is introduced percutaneously. From a total of 23 occlusive arteriosclerotic vascular disease cases, about 50% showed a marked acidosis at below knee amputation level with normal pH in the thigh. Our examinations, though restricted in number, have born out our assumption that provided pH in the muscle is normal, the amputation stump may be expected to heal, whereas tissue acidosis will lead to necrosis. In one third of the patients pH measurement suggested an amputation level different from that determined by clinical examination.
Based on our experience so far, tissue pH measurement has provided a new method, which, when used in conjunction with clinical findings, is of value when making the initial decision concerning level of amputation.
Zusammenfassung
Die Wahl der Amputationshöhe bei chronischen arteriellen Durchblutungsstörungen allein nach klinischen Gesichtspunkten ist unbefriedigend. Während die Durchblutung von Haut und Subcutis gut beurteilt werden kann, sind die Durchblutungsverhältnisse in der Muskulatur schwierig zu erfassen. Objektive Untersuchungsmethoden bieten — mit Ausnahme der Arteriographie in einzelnen Fallen — keine Hilfe bei dieser Fragestellung. p ]In der Messung der Wasserstoffionenkonzentration in der Muskulatur liegt die Möglichkeit, eine Prägangrän zu erkennen, da so die vermehrte Lactatbildung auf Grund eines anaeroben Stoffwechsels nachgewiesen werden kann.
Zur pH-Messung im Gewebe wurde eine durch eine Metallkanüle geschützte Glaselektrode verwendet, die percutan eingeführt wird. Bei 23 Patienten mit chronischen arteriellen Durchblutungsstörungen wurde in der Hälfte der Fälle eine deutliche Acidose auf Unterschenkelamputationshöhe bei normalem pH im Oberschenkel festgestellt. Unsere zahlenmäßig beschränkten Untersuchungen bestätigten die Annahme, daß bei normalem pH in der Muskulatur eine Heilung des Amputationsstumpfes erwartet werden kann, während es bei Gewebsacidose zur Nekrosebildung kommt. Bei einem Drittel der Patienten ergab die pH-Messung eine andere Beurteilung der Amputationshöhe als die klinische Untersuchung.
Die bisherigen Erfahrungen zeigen, daß in der Gewebe-pH-Messung eine neue Untersuchungsmethode zur Verfügung steht, um zusammen mit der klinischen Beurteilung von Anfang an die geeignetste Amputationshöhe zu wählen.
