Abstract
Operating problems associated with existing capillary membrane oxygenators have been impaired CO2 removal, nonuniform tube perfusion, thrombus formation in the blood manifolds (headers) and fall-off in performance with scale-up of small units. These faults are primarily due to poor tube ventilation, inadequate control of tube lengths, and ineffective header design. A novel semiautomatic fabrication technique is described that minimises or eliminates these deficiencies. The technique produces a well ventilated tube bundle by accurately spacing the tubes within the potted end-sections (tube sheet). The tube sheet is 10–12 mm thick, about one half of that in previously described units. To give uniform tube perfusion and eliminate stagnant regions over the tube sheet, blood is introduced normal to the tube axis through headers of hemitoroidal configuration. Three oxygenators with different dimensions and flow capacities were testedin vitro andin vivo. Good agreement was obtained between the theoretically predicted and experimentally measured oxygen-transfer rates for all units, no loss in performance being shown with oxygenator scale-up. After canine partial-bypass of six hours duration, thrombus formation was minimal in the headers with only 1 per cent of the tubes being occluded. Further optimisation is possible with smaller-diameter capillaries.

Les problèmes de fonctionnement associés aux oxygénateurs de membrane capillaire fabriqués jusqu'ici consistaient en une mauvaise élimination du CO2, une perfusion tubulaire non uniforme, la formation de caillots dans les replis (embouts coudés) et la baisse de rendement avec la formation de dépôt dans les petites unités. Ces fautes sont essentiellement dues à une mauvaise ventilation du tube, à un réglage imparfait de la longueur des tubes et à une conception inefficace des embouts collecteurs. La nouvelle technique de fabrication semi-automatique décrite minimise ou supprime ces imperfections. Cette technique permet de produire un faisceau de tubes bien ventilé grâce à un espacement précis des tubes au niveau des sections terminales renflées (plaque tubulaire). La plaque tubulaire a une épaisseur de 10 à 12 mm, soit environ la moitié de celle des ensembles préalablement décrits. Afin d'obtenir une perfusion uniforme et d'éliminer les zones de stagnation dans la plaque tubulaire, le sang est introduit perpendiculairement à l'axe du tube au moyen d'embouts de configuration semitoroidale. Trois oxygénateurs de dimensions et de débit différents ont été testésin vitro etin vivo. Ces tests ont montré une bonne concordance pour les trois modèles entre les rythmes de transfert d'oxygène prévus en théories et ces mêmes rythmes mesurés expérimentalement, aucune perte de rendement due à la formation de dépôt dans l'oxygénateur n'ayant été enregistrée. Au bout de six heures de dérivation partielle pratiquée sur des chiens, la formation de caillots était minime dans les embouts coudés, 1% seulement des tube étant bouché. Des capillaires de diamètre plus petit permettraient une amélioration supplémentaire.

Betriebsprobleme bestehender Kapillarmembran-Sauerstoffanreichergeräte umfassen Beeinträchtigung der CO2-Beseitigung, uneinheitliche Röhrendurchströmung, Thrombenbildung in den Blutverteilern und Leistungsabfall bei der maßstäblichen Vergrößerung von kleinen Einheiten. Diese Fehler liegen in erster Linie bei der schlechten Röhrenentlüftung, ungenügenden Regelung der Röhrenlänge und beim ungünstig konstruierten Verteiler. Die hier beschriebene neue halbautomatische Fertigungstechnik reduziert diese Fehler auf ein Minimum oder schaltet sie ganz aus. Das Verfahren ergibt ein gut gelüftetes Röhrenbündel, indem es die Röhren innerhalb der vergossenen Endabschnitte in genauen Abständen anordnet (Röhrenwand). Die Röhrenwand ist 10 bis 12 mm dick, etwa die Hälfte von früher beschriebenen Einheiten. Zum Zweck einheitlicher Röhrendurchströmung und der Ausschaltung von Stauzonen in der Röhrenwand wird Blut senkrecht zur Röhrenaxis durch Verteiler von halbtoroider Anordnung eingeführt. Drei Sauerstoffanreichergeräte verschiedener Abmessungen und Durchflußkapazität wurdenin vitro undin vivo geprüft. Die theoretische vorhergesagte Sauerstoffübertragungsgeschwindigkeit aller Einheiten stimmte mit der experimentell gemessenen gut überein, und bei der maßstäblichen Vergrößerung trat kein Leistungsabfall auf. Nach sechsstündigem Teilumlauf war die Thrombenbildung in den Verteilern minimal, und nur 1% der Röhren blockiert. Kapillarröhren mit kleinerem Durchmesser können weiterhin verbessert werden.
