Leiomyoblastoma of the duodenum masquerading as carcinoma of the head of the pancreas

Abstract

We report a case of leiomyoblastoma in the duodenum masquerading as carcinoma of the head of the pancreas in a 74-year-old Japanese man. The patient had no abdominal symptoms. A hard mass was palpated in the right upper quadrant. Ultrasonography demonstrated an irregular hyperechoic mass in the head of the pancreas. Computed tomography showed a mass of irregular density at the same anatomical site. Angiography displayed a hypervascular lesion with encasement of the pancreatoduodenal arcades. A laparotomy was performed, with the tentative preoperative diagnosis being carcinoma of the head of the pancreas. A standard pancreatoduodenectomy was performed. However, the final histopathologic diagnosis of the resected specimen proved to be leiomyoblastoma of the duodenum proliferating extraluminally into the head of the pancreas. We briefly discuss the clinical implications of duodenal leiomyoblastoma.
