Zusammenfassung
Mit der Methode der Glucose-1-14C-Oxydation zu14CO2 am epididymalen Fettgewebe der Ratte haben wir die insulinähnliche Aktivität (ILA) im Serum bei zwei Gruppen extrem adipöser Patienten (91 und 64% Übergewicht) und bei Normalpersonen vergleichend vor und nach oraler Glucosebelastung bestimmt. Von den beiden Gruppen der Adipösen umfaßte die erste ausschließlich Personen ohne familiäre Belastung mit Diabetes und ohne nachweisbare Störung der Kohlenhydrattoleranz, die zweite Patienten mit familiärer Belastung mit Diabetes und teilweise pathologischer Glucosetoleranz. Jeder ILA-Wert wurde als Mittelwert aus sechs Einzelbestimmungen errechnet.
Bei extremer Adipositas war die ILA im Nüchternserum im Mittel stark erhöht. Die Insulinreserve war bei der ersten Gruppe der Fettsüchtigen unverändert, bei der zweiten Gruppe deutlich vermindert. Die Blutzuckerwerte erreichten im Mittel bei beiden Gruppen im Gegensatz zu Normalen erst nach 60 min den Maximalwert.
Der Hyperinsulinismus der Adipositas ist offenbar adaptiv, ein Anhalt für seine Priorität bestcht nicht. Möglicherweise kann es durch Erschöpfung des B-Zell-systems zum relativen Insulinmangel und zum Diabetes mellitus kommen. Der Übergang von der Adipositas mit herabgesetzter Glucosetoleranz zum Altersdiabetes mit Fettsucht ist nach dieser Vorstellung fließend. Die möglichen Ursachen der offenbar bei der Fettsucht bestehenden Insulinunterempfindlichkeit werden diskutiert.
Nach unseren Befunden muß bei Untersuchungen der ILA bei Diabetikern das Körpergewicht der Patienten immer berücksïchtigt werden.
Summary
Using the method of glucose-1-14C oxydation to14CO2 on the rat epididymal adipose tissue, the insulin-like activities (ILA) in the serum have been compared before and after oral loading with glucose in two groups of extremely obese patients (91 and 64% overweight) and in normal subjects. The first group of the obese consisted of subjects without known diabetic heredity and with normal response to glucose tolerance tests; the subjects of the second group with known diabetic heredity showed in part impairment of the glucose tolerance. Each ILA-value was calculated from the mean values of 6 individual determinations.
The mean fasting values of the ILA of the extremely obese patients were found to be markedly increased. The insulin reserve was unchanged in the first group of the obese subjects and distinctly reduced in the second group. In contrast to normal subjects the mean blood sugar values in both groups reached their maximum values only after 60 minutes.
Apparently there is an adaptive hyperinsulinism in obesity without any support for its priority. Possibly an exhaustion of the B-cell-system can lead to the manifestation of diabetes mellitus. According to this conception the passing over from the obesity with impaired glucose tolerance to maturity-onset diabetes with obesity is gradual. The possible causes of the apparently existing insensitivity to insulin are discussed.
According to our results in the determination of the ILA in the serum of diabetics one must always pay attention to the patients weight.
