Zusammenfassung
Alle Patienten mit Wohnsitz im Sektor der Psychiatrischen Klinik der Medizinischen Hochschule Hannover, die wegen einer Alkoholkrankheit oder einer Medikamentenabhängigkeit erstmals hospitalisiert und in der Zeit vom 1. Januar 1973 bis 31. Dezember 1978 entlassen worden waren, wurden bis zum 31. Dezember 1979 nachbeobachtet. Wichtigste Determinante der Rehospitalisierungswahrscheinlichkeit war das Alter der Patienten. Patienten im Alter bis zu 40 Jahren wurden häufiger rehospitalisiert als Patienten über 40. Geschlechtsunterschiede in der psychiatrischen Institutionskarriere von Alkoholkranken waren klein und inkonsistent. Eine relativ grosse Hospitalisierungswahrscheinlichkeit für beruflich besonders qualifizierte Männer und Frauen war auffällig, allerdings statistisch nicht signifikant. Zwar könnte sie auch durch Alterseinflüsse bedingt sein, weist im Kontext anderer Untersuchungen jedoch daraufhin, dass gesellschaftliche Rollenerwartungen psychiatrische Institutionskarrieren von Alkoholpatienten determinieren.
Summary
All patients residing in the catchment area (“sector”) of the Psychiatric Clinic of Hanover Medical School who had been hospitalized due to alcoholism or drug dependence for the first time in their lifes and had been discharged over the period January 1, 1973, to December 31, 1978, were followed through December 31, 1979. Age was the major determinant of rehospitalization. Patients up to age 40 were rehospitalized more frequently than patients 40 and over. Sex differences in the psychiatric institutional career of alcoholics were small and inconsistent. A relatively large probability of rehospitalization was noted for men and women with high occupational status, failed to reach significance, however. This might be due to confounding by age but together with results of other studies also supports the notion that societal role expectations determine the psychiatric institutional careers of alcoholics.
Résumé
Tous les patients habitant dans le secteur de la clinique psychiatrique de la Medizinische Hochschule Hannover ayant été hospitalisés pour la première fois en raison d'alcoolisme ou abus de médicaments et qui sont sortis entre le 1er janvier 1973 et le 31 décembre 1978 ont été suivis jusqu'au 31 décembre 1979. L'âge des patients était le fait le plus important déterminant la probabilité de réhospitalisation. Les patients de moins de 40 ans ont été réhospitalisés plus souvent que les patients de plus de 40. Les différences selon le sexe dans la «carrière» psychiatrique en institution étaient minimes et inconsistantes. On constate une probabilité frappante d'hospitalisation relativement élevée chez les hommes et femmes à profession hautement qualifiée, mais qui n'était pas significative au point de vue statistique. Cette probabilité pourrait, il est vrai, être aussi déterminée par des influences dues à i'âge, mais dans le contexte d'autres études elle indique que les attentes sociales de rôle déterminent les «carrières» psychiatriques en institution des patients alcooliques.
