Evaluationsansätze tagesklinischer Behandlung in der Gerontopsychiatrie

Zusammenfassung

Ausgehend von den international sehr unterschiedlichen Konzeptionen und damit auch Indikationsstellungen für gerontopsychiatrische Tageskliniken wird in einer Reihe von drei Untersuchungen versucht, den Stellenwert einer in eine psychiatrische Klinik organisatorisch eingegliederten Tagesklinik in der BRD für die gesamte psychiatrische Versorgung alter Menschen aufzuzeigen. Damit ergeben sich auch Klärungen für die Indikation dieser Behandlungsform, die deutlich über das hinausgehen, was zum Beispiel in Grossbritannien und in den Niederlanden als Indikationsbereich angesehen wird. So ist es in diesem Setting möglich, nicht nur Patienten mit unterschiedlich stark ausgeprägter Demenz zu versorgen, sondern auch vor allem leichte bis mittelschwere affektive und schizophrene Psychosen. Die gerontopsychiatrische Tagesklinik kann damit als ein notwendiges Glied in der Kette der psychiatrischen Versorgung alter Menschen ausgewiesen werden.

Résumé

Dans plusieurs pays, les expériences faites avec les hôpitaux de jour pour patients âgés ont apporté différents points de vue, concernant les indications spécifiques pour ce traitement particulier. Par plusieurs investigations, on a essayé de clarifier la position d'un hôpital de jour pour personnes âgées, intégré dans un hôpital psychiatrique. Ainsi, les indications pour cette forme de traitement ont pu être précisés et même élargies, plus que les résumés publiés à ces jours dans la littérature britannique ou néerlandaise l'ont fait paraître. Dans ce concept, il est non seulement possible de traiter les personnes présentant une démence plus ou moins grave, mais aussi celles qui souffrent d'une légère à moyenne psychose affective ou schizophrène. L'hôpital gérontopsychiatrique de jour trouve ainsi sa place dans la chaîne de traitement psychiatrique des personnes âgées.

Summary

Experiences with day hospitals for elderly patients in various countries have led to differing views regarding specific indications for this particular treatment mode. In a series of investigations we tried to clarify the place of a day hospital for elderly patients which is an integrated part of a psychiatric hospital within the total of psychiatric care of the elderly. As a result we are able to demonstrate a spectrum of indications for psychogeriatric day hospital treatment far broader than mentioned in the majority of the relevant literature of the UK and the Netherlands e. g. In this particular setting it is possible to give treatment not only to patients with dementia of differing degree but also to mild or moderately disturbed affective or schizophrene psychoses. Thus the psychogeriatric day hospital can be demonstrated to be a necessary link within the chain of psychiatric care of the elderly.
