Zusammenfassung
Mit Hilfe spiralcomputertomographischer Untersuchungen einschließlich der Anfertigung von Sekundärrekonstruktionen in multiplen Orientierungen anstelle der klassischen Röntgenkontrolle mittels Orthopantomogramm und Altschulaufnahme sollten die behandlungsinduzierten Veränderungen wie Aufrichtung oder Resorption des kleinen Fragments, Remodellierung und die Beteiligung der angrenzenden Strukturen Fossa und Eminentia articularis sowie des Jochbogens nach sechsmonatiger funktioneller Behandlung mit dem Aktivator quantitativ und qualitativ erfaßt werden. Nur vier der 16 traumatisierten Kiefergelenke (von zwölf zum Traumazeitpunkt zwischen fünf und 16 Jahre alte Patienten) zeigten trotz guter bis sehr guter funktioneller Wiederherstellung einmal eine gute bzw. eine moderate Remodellierung (drei). Die quantitative und qualitative Analyse der 2-D-und 3-D-Rekonstruktionen ergab drei hypoplastische, fünf gering und fünf weitere ausgeprägt verplumpte Kondylen. Sieben Fragmente waren teilweise resorbiert, zwei hatten sich aufgerichtet, vier Kiefergelenke zeigten freie Fragmente, und drei hatten Neoarthrosen, beispielsweise mit der Eminentia articularis, gebildet. Es wird ein Zusammenhang zwischen dem Frakturtyp und der Güte der Remodellierung vermutet. Die vorliegenden Resultate erlauben zumindest eine Hypothese der Ätiologie der unter dem „Diktat” der Funktion stattfindenden Neubildung des Gelenkkopfs und Gelenkfortsatzes. Exzessive Knochenapposition nach Anteriorverlagerung scheint eine wichtige Rolle bei den unerwünschten Überentwicklungen der Kiefergelenkköpfe zu spielen.
Summary
In this retrospective follow-up study after 6 months of treatment with functional appliance therapy of 12 children who had suffered condylar fractures, spiral CT was employed for the radiological examination with 2D and 3D reconstructions. Although all patients exhibited restoration of normal function, only 4 showed favorable remodeling. The other patients had developed 5 hypoplastic, 5 moderate and 5 severe deformed condyles. Seven condylar fragments were resorbed; 2 were uprighted; 4 had developed bony spurs and 3 neoarthrosis. The results revealed a fracture dependent remodeling. The authors discuss the etiology of the functional stimulated new bone formation of the condylar process. Excessive bone apposition as a reaction to the anterior shift of the mandible seems to play an important role during the overgrowth of the head of the condyle.
