Bleeding duodenal diverticulum

Summary

1. Major gastrointestinal bleeding is a serious but infrequent complication of duodenal diverticula.

2. Diverticula of the third (horizontal) and fourth (ascending) portions of the duodenum at Treitz's angle figure conspicuously in the collected case histories of bleeding. This predilection is explained by the great vascularity of the diverticular bed near Treitz's angle.

3. Diverticula of the third and fourth portions of the duodenum should, if symptomatic, be operated upon. They can be excised with safety. The surgical approach to the diverticula of the third and fourth portions is outlined briefly.

4. The case history of a large bilobed diverticulum of the ascending portion of the duodenum near Treitz's angle is reported. The diverticulum was associated with recurring melena, which required in the course of 15 months 21 blood transfusions. The bleeding did not recur after resection of the diverticulum.
