Zusammenfassung
Die Gnathologie hat in gewissen Kreisen der rekonstruktiven Zahnheilkunde zunehmend Verbreitung gefunden. Die Forderungen dieser Lehre werden neuerdings in vermehrtem Maße auch an das kieferorthopädische Fachgebiet herangetragen. Die Position des Unterkiefers in zentrischer Relation (Centric relation) ist grundlegender Ausgangspunkt des gnathologischen Konzeptes. In einer Übersicht über das kephalofaziale Wachstumsgeschehen wird dargelegt, daß die determinierende Unterkieferlage für die Okklusionsfindung während der gesamten Entwicklungsphase nicht im Grenzbereich der Scharnierachsenbewegung (Hinge axis) liegen kann. In der klinischen Tätigkeit bedarf aber der Kieferorthopäde einer okklusionsunabhängigen Referenzposition des Unterkiefers, um funktionelle Abweichungen diagnostizieren, eine ständige Ortung während der funktionell desorientierenden Behandlungsphasen durchführen und das Behandlungsergebnis überprüfen zu können. In dieser Hinsicht vermag die Retrusionskontaktstellung (Most retruded contact position) gute Dienste zu leisten, obwohl sie die Forderung einer idealen Fossa-Kondylus-Beziehung nicht ganz erfüllt. Deshalb darf sie, unter gewissen Vorbehalten, auch nur als eine diagnostischeAusgangsstellung qualifiziert werden. Es wäre falsch-wie am dargelegten Fallmaterial demonstriert-dieser terminalen Stellung eineabsolute Bezugsrolle zuzuweisen, auf die unter allen Umständen die ganze okklusoartikuläre Dynamik ausgerichtet werden muß.
Summary
Gnathology has gained wide acceptance in some circles of reconstructive dentistry. Increasingly, the requirements of this philosophy are imposed upon the orthodontic branch. Mandibular position in centric relation is the very basis of the gnathological concept. In an overview of cephalo-facial growth, it has been demonstrated, however, that the determining mandibular position for occlusal orientation during the entire developmental period cannot be located along the border range of the hinge axis. In this clinical endeavours, the orthodontist needs a mandibular reference position independent of occlusion to be able to diagnose functional deviations and to continuously assess the maxillo-mandibular relationship during the functionally desorienting treatment phases, at the conclusion and after treatment. In this respect, the most retruded contact position has great merits although the requirement of ideal fossa-condylerelationship is not entirely satisfied. Therefore, centric relation qualifies, with some reservations, only for being a diagnosticstarting position. It would be misjudgement, however, — as demonstrated by the presented cases — to assign to this terminal mandibular position anabsolute status of reference from and to which, without restrictions, all dynamics of occlusion have to be directly built.
Résumé
La gnathologie a trouvé un intérêt croissant dans certains domaines de la dentisterie restaurative. Les exigences de cette science s'imposent également de plus en plus en orthopédie dento-faciale. La position de la mandibule en relation centrée (centric relation) est l'élément de base du concept gnathologique. Un survol des phénomènes de croissance crânio-faciale démontre que la position de la mandibule déterminante pour l'orientation occlusale durant toute la période de développement, ne peut se situer dans les seules limites du mouvement en axe charnière (hinge axis). Dans sa pratique, l'orthodontiste a besoin d'une position de référence de la mandibule qui soit indépendante de l'occlusion afin de pouvoir diagnostiquer des troubles fonctionnels, de pouvoir évaluer la relation mandibulo-maxillaire, durant la phase de désorientation fonctionnelle, à la fin du traitement et après ce dernier. De ce point de vue, la position de contact en rétrusion (most retruded contact position) a de grands mérites, même si la nécessité d'une relation idéale du condyle dans sa cavité glénoide n'est pas entièrement satisfaite. Pour cette raison et avec quelques réserves, la relation centrée n'est qu'uneposition de départ lorsque l'on pose un diagnostic. Il serait donc erroné comme le prouvent les cas présentés, d'assigner à la position axiale terminale de la mandibule unstatus absolu de référence par rapport auquel, sans restriction, toute la dynamique de l'occlusion devrait être érigée.
