Tamoxifen und Endometrium: Überwachung und abklärung endometrialer veränderungen

Zusammenfassung

Dem Umstand, daß Tamoxifen die Entstehung von malignen Endometriumveränderungen begünstigt, muß insbesondere im Rahmen von Präventionsstudien besonders Rechnung getragen werden. In erster Linie bietet sich zur Überwachung des Endometriums der vaginale Ultraschall vor Tamoxifen-Beginn und anschließend in etwa jährlichen Abständen an. Bei einer Verbreiterung des Endometriums über 8 mm oder bei Auftreten einer vaginalen Blutung wird die histologische Abklärung empfohlen. Wenn in der Curettage inadäquates Material gefördert wird, ist eine hysteroskopische Endometriumresektion zu empfehlen. Zur Differenzierung zwischen einfacher Endometriumveränderung und (abklärungsbediirftigen) Endometriumpolypen kann die Hydrosonographie auch schon bei einer Verbreiterung zwischen 5–8 mm herangezogen werden. Wir empfehlen die Weiterführung der sonographischen Endometriumkontrollen über mindestens 10 Jahre nach Absetzen der Tamoxifen-Verabreichung, weil Endometriumkarzinome auch Jahre nach Absetzen der Tamoxifen-Therapie noch gehäuft auftreten.
