Zusammenfassung
Im ersten, tierexperimentellen Teil dieser Abhandlung wurde der Einfluß von lang andauernder orthodontischer Kraftapplikation auf die Quantität des peri- und paraimplantären Knochens sowie den direkten Knochenkontakt (%) an der Implantatoberfläche bei Titanimplantaten (Brånemark, Dimension: 10×3,75 mm) überprüft. Dazu wurden bei zwei Foxhounds jeweils sechs Prämolaren extrahiert (P3 Mandibula/Maxilla und P4 Mandibula). Nach zwölf Wochen erfolgte die Insertion von sechs Implantaten pro Hund (P3- bzw. P4-Bereiche). Die Fixturen in den P3-Bereichen dienten nach einer Implantatheilungsphase von 25 Wochen als Verankerungselemente (Testimplantate) zur Distalisierung der P2 über 26 Wochen (kontinuierliche Kraftgröße: ∼2 N). Die Implantate in den P4-Bereichen der Mandibula bildeten eine Kontrollgruppe. Die histologische Analyse ergab bei den Testimplanaten (Mandibula) im Vergleich zu den Kontrollimplantaten (Mandibula) folgende Unterschiede: breitere Kortikalis, breiterer periimplantärer Knochenmantel, dichtere, teils sklerotische Spongiosa (Druckseite) sowie dickere Trabekel (Zugseite). Die fluoreszenzmikroskopische Analyse ergab, daß diese Befunde durch ausgeprägte lamelläre Knochenappositionen in der Kraftapplikationsphase auftraten. Im Vergleich dazu konnten bei den Testimplantaten (Maxilla) weitaus geringere lamelläre Appositionen nachgewiesen werden. Der mittlere Prozentsatz an direktem Knochenkontakt an der Implantatoberfläche betrug: Kontrollimplantate (Mandibula, n=4): 40,5%; Testimplantate (Mandibula, n=4): 51,6%; Testimplantate (Maxilla, n=4): 42,4%. Im zweiten, klinischen Teil werden Indikationen und Möglichkeiten der Nutzung enossaler Implantate als orthodontische Verankerungselemente dargestellt: erstens zur orthodontisch-prothetischen Verankerung und zweitens zur ausschließlich orthodontischen Verankerung. Die Bedeutung dieser Aspekte für den Implantationsort, die Implantatdimension und den Typ der Implantatverankerung werden aufgezeigt.
Summary
Part 1 of this study, which deals with experimental use of titanium implants in animals (Brånemark, dimension 10×3.75 mm), examines the long-term orthodontic application of force on the quantity of periimplantary and paraimplantary bone and on direct bone contact (in %) at the implant surface. In the experiment 6 premolars (P3 mandible/maxilla and P4 mandible) were extracted from each of 2 foxhounds. After 12 weeks each dog received 6 implants (P3 and P4 areas). After the implants had healed for 25 weeks, the fixtures in the P3 areas were used as anchoring elements (test implants) for P2 distalization over a period of 26 weeks (continuous force applied: ∼2 N). The implants in the P4 area served as the control group. Histological analysis revealed in the test implants (mandible) in comparison to the control implants (mandible) the following differences: broader cortical bone; wider periimplantary bone casing; denser, partly sclerotic spongiosa (pressure side); and thicker trabecula (tension side). Fluorescence microscopy revealed that these findings were the result of distinct lamellar bone apposition during the force application phase. In comparison, far less lamellar apposition was found in the test implants (maxilla).
