Von der Orthodontie zur Mundorthopädie

Zusammenfassung

1910 Angle war Alleinherrscher, sein Lehrbuch war Gesetz.

1915 Ein Aufsatz aus meiner Hand, wo betont wird, daß die Stellungnahme von Angle, die Extraktion sei zu jeder Zeit unnötig, als biologisch falsch zurückgewiesen werden mußte.

1928 Ein aufsatz „Abnehmbare Apparate in der Kieferorthopädie”. Ungefähr zur gleichen Zeit publizierte Andresen seine „Funktionelle Kieferorthopädie”. Seitdem wurden in Europa mehr und mehr abnehmbare Apparate benutzt.

In Amrika wurde inzwischen die Anglesche Apparatur verbessert, und es wurden neue Methoden mit festsitzenden Apparaten publiziert. Haute hat man in Europa wieder Interesse für diese Apparate (Edgewise, Begg usw.), während in Amerika die abnehmbaren Apparate mehr und mehr benutzt werden. Meines Erachtens fehlt der Beweis, daß festsitzende Apparate in bestimmten Fällen notwendig sind. Diese Apparate haben den großen Nachteil, daß man dann zu spät anfängt; man behandelt vernachlässigte Fälle.

Moderne Mundorthopädie bedeutet Vorbeugung, d. h. Eliminieren — so viel wie möglich — aller exogenen Faktoren, die die normale Entwicklung des Wachstums des Gesichts beeinflussen. Es bedeutet auch: sofortige Behandlung, nachdem Anomalien diagnostiziert sind.

Die moderne orthodontische Philosophie soll sich nicht länger festbeißen in Kephalometrie, Modellen und Apparaten, man sollte realisieren, daß es sich hier nicht so sehr um Zahnheilkunde handelt, als um einen wichtigen Teil der allgemeinen Orthopädie.

In 1915 I published an article in the Dental Cosmos, in which I stated that Angle's point of view that extraction of teeth was always a mistake in orthodontic treatment, was biologically untenable.

In 1928 I wrote an article on “Loose appliances in Orthodontia”. about the same time Prof. Andresen (Copenhague) published his “Functional Jaworthopedics”. From that time on in Europe loose appliances were used more and more, while in the U.S.A. Angle's appliances were improved and new full band appliances were propagated (Edgewise, Begg etc.). To-day there is in Europe an awakened interest in these fixed appliances, while in America the loose appliances and preventive orthodontics in general are becoming more popular.

I am of the opinion that it is not provent that full band appliances are necassary and they have the serious disadvantage that then treatment is started too late, so that one treats neglected cases; is very expensive and therefore practically impossible in social orthodontics.

Mouth Orthopedics to-day means prevention, that is: elimination from birth onwards, as much as possible, of all the exogenic factors influencing the normal growth of the face. It also means early treatment, as soon as anomalies are diagnosed.

Modern orthodontic philosophy should no longer only think in cephalometrics, patterns and appliances, but should realize that this specialty is an important part of General Orthopedics, so that patients should be treated along orthopedic lines.
