Zusammenfassung
Bei 38 Patienten wurden 536 Metall- und Plastikbrackets mit einem Akrylat-Epoxidharzadhäsiv aufgeklebt. Über einen Beobachtungszeitraum von 13,28 Monaten betrug die Bruchhäufigkeit 7,46%. Die Verlustrate war im Seitenzahnbereich signifikant höher als im Frontzahnbereich und bei Metallbrackets höher als bei Plastikbrackets.
Mit Beginn der orthodontischen Behandlung zeigte sich durch die entstandenen Retentionsnischen ein signifikanter Anstieg des Plaquebefalls und der gingivalen Entzündung an den Fazial- und vornehmlich an den Approximalflächen. Die Oralflächen der Zähne, als Vergleichsparameter, blieben weitgehend unbeeinflußt.
An 105 Zähnen konnten bei der Kontrolluntersuchung die, Brackets entfernt werden. Als Folge der Plaqueretention wiesen 17,1% der Zähne stellenweise linienförmige kreidige Schmelzverfärbungen auf, in zwei Fällen waren kariöse Defekte entstanden. Diese Befunde unterstreichen die zentrale Bedeutung der Plaquekontrolle bei der Auswahl und Instruktion der Patienten sowie während der Behandlung. An 13,33% der Zähne wurden nach der Bracketentfernung flächige Schmelzausrisse festgestellt, die als potentielle Nebeneffekte der Adhäsivtechnik in Kauf genommen werden müssen.
Summary
536 metal and plastic brackets were bonded with an acrylic-epoxy resin adhesive on 38 patients. During an observation period of 13,28 months 7,46% of the brackets came off. The rate of failure was significantly higher in the premolar and molar region as compared to the front and with metal brackets as compared to plastic ones.
With the beginning of the orthodontic treatment, a significant increase of the plaque accumulation and gingival inflammation was found, especially at the facial and approximal tooth surfaces. The oral surfaces showed no significant changes.
On 105 teeth the brackets were removed at the end of the observation period. Due to the plaque retention 17,1% of these teeth showed small localized white spots, in two cases carious enamel defects had formed. These findings emphasize the importance of plaque control for selection, instruction and care of orthodontic patients. After debonding, localized enamel defects were seen on 13,33% of the examined teeth, but these lesions must be accepted as potential side-effects of the direct bonding technique.
Résumé
Chez 38 patients, on a collé à la résine adhésive 536 brackets métalliques et en plastique. Pendant une période d'observation de 13,28 mois, 7,46% des brackets se sont détachés. Leur descellement était plus fréquent au niveau des dents latérales qu'antérieures et pour les brackets métalliques que pour ceux en plastique.
Au début du traitement orthodontique, on a constaté une augmentation significative de la plaque dentaire et de l'inflammation gingivale, particulièrement au niveau des surfaces vestibulaires et proximales des dents. En comparaison les surfaces dentaires linguales ne montraient pas de différence significative.
Au moment du contrôle, il a été possible d'ôter les brackets sur 105 dents. A cause de la plaque dentaire, 17,1% des dents présentaient par endroit des lignes de déminéralisation crayeuses, voire dans deux cas de la carie. Ces constatations soulignent l'importance du contrôle de la plaque quand il s'agit d'envisager un traitement et d'instruire des patients en orthodontie. Après dépose des brackets collés à la résine, on a trouvé des arrachements superficiels d'émail chez 13,33% des dents examinées; c'est un inconvénient dont il faudra tenir compte lorsqu'on utilise la technique adhésive.
