Zusammenfassung
Die autogene Zahntransplantation ist eine alternative Behandlungsmethode zur Rekonstruktion der Kontinuität des Zahnbogens nach vorzeitigem Zahnverlust. Die Differentialindikation zwischen Transplantation und anderen Verfahren zum Lückenschluß wird von der Prognose der Transplantate bestimmt. Transplantierte Keime oberer Prämolaren und unterer dritter Molaren vollenden das Wurzelwachstum. Die vorgelegte klinische und röntgenologische Analyse von 81 autogen transplantierten oberen Eckzähnen nach unterschiedlichen Operationsverfahren weist aus, daß 50% der Transplantate fünf Jahre und nur 25% länger als sechs Jahre in situ verbleiben. Die statistischen Überlebenschancen vital verpflanzter, apektomierter Zähne sind günstiger. Die Frühzeitprognose wird durch entzündliche Knochen-und/oder Zahnresorptionen infolge odontogener Infektion oder Okklusionstrauma, die Langzeitprognose durch unterschiedliche Zahnresorptionen bestimmt. Durch regelmäßige röntgenologische Nachkontrollen können odontogene Infektionen frühzeitig erkannt und endodontisch behandelt werden. Die zweifelhafte Prognose mahnt zu strenger Indikation.
Summary
Autogenous tooth transplantation is an alternative treatment for preserving continuity of the dental arch following early loss of teeth. The prognosis of transplants determines the indication as compared with other methods of space closure. Transplanted buds of upper premolars as well as of lower third molars continue their root growth. The clinical and roentgenological analysis of 81 autogenously transplanted upper canines, following different operative procedures, indicates, that 50% of the transplants remain in situ for five years and only 25% remain in situ longer than six years. The statistical chances of survival of vitally transplanted, apicected teeth are more favourable. Short-term prognosis is determined by inflammatory bone and/or tooth resorption due to odontogenous infection or occlusal trauma. Long-time prognosis is determined by different forms of tooth resorption. Regular radiographic examination as reveal early odontogenous infections which can then be treated endodontically. The doubtful prognosis necessitates strict indicatons.
Résumé
La méthode de la transplantation dentaire autogène est une alternative pour rétablir la continuité de l'arcade dentaire dans le cas d'une perte précoce des dents. C'est le pronostic des transplants qui détermine l'indication différenciée entre la transplantation et d'autres procédés pour colmater la brèche. Lors de la transplantation, des germes de prémolaires supérieures et de troisièmes molaires inférieures achèvent la croissance de leurs racines. La présente analyse clinique et radiologique nous apprend que sur 81 canines supérieures transplantées par voie autogène selon des procédés différents, 50% restent en place pendant cinq ans et 25% seulement pendant plus de six ans. Selon les statistiques, les chances de survie sont meilleures pour les dents transplantées vivantes et apectomisées. Le pronostic à court terme est influencé par des résorptions inflammatoires de la dent et/ou de l'os qui proviennent d'une infection odontogène ou d'une lésion par occlusion. Le pronostic à long terme dépend de diverses résorptions des dents. A l'aide d'examens ultérieurs radiologiques réalisés de façon régulière, les infections odontogènes peuvent être dépistées tôt et soignées e manière endodontique. Un pronistic incertain requiert une indication sérieuse.
