Zur Frage der Wachstumsinduktion durch Kondylusrekonstruktion beim Kind

Zusammenfassung

In sechs Fällen schwerer einseitiger Minderentwicklung des Unterkiefers im Kindesalter aufgrund einer Agenesie, Hypoplasie oder Ankylose wurden autologe osteochondrale Transplantate zum Ersatz des Unterkieferkondylus verpflanzt. Das Alter betrug vier bis 14 Jahre, die Beobachtungszeit ein bis drei Jahre. Postoperativ wurden funktionskieferorthopädische Geräte oder Plattenapparaturen eingesetzt. In allen Fällen ließ sich eine gute bis sehr gute schmerzfreie Funktion erzielen, ebenso fanden sich in allen Fällen Hinweise auf Wachstum im Transplantatbereich. In einem Fall kam überschießendes Wachstum zustande. Anhand der Fälle werden die interdisziplinäre Indikationsstellung und Planung sowie die postoperative kieferorthopädische Therapie diskutiert.

Summary

Six children, presenting hypoplasia, ankylosis or angenesis of the temporomandibular joint, underwent autologous reconstruction of the condyle. At the time of surgery, the patients were at an age for four to 14 years. Postoperative follow-ups continued the following one to three years, whereby removable orthodontic appliances were used. In all cases postoperative function varied from good to very good and was free of pain. Additionally, in three cases, signs of growth in the area of transplantation were successively achieved. One case showed excessive growth. Regarding these cases of condylar hypoplasia, the rationale for the indication of surgical treatment as well as postoperative orthodontic treatment are discussed.
