Zusammenfassung
Bei 120 11jährigen kieferorthopädischen Patienten wurden die orofazialen Dyskinesien: inkompetente Lippenhaltung, einfaches Zungenpressen und viszerale Schluckart auf ihre pathologischen HNO-ärztlichen Befunde untersucht. Die Ergebnisse wurden den Befunden bei Patienten mit offenem Biß bzw. mit horizontalem und vertikalem Gesichtsschädelaufbau gegenübergestellt. Nach den rhinologischen und rhinomanometrischen Untersuchungen war bei Probanden mit inkompetenter Lipenhaltung und einfacher Zungendyskinesie die Nasenatmung häufiger beeinträchtigt als bei Patienten mit offenem Biß und vertikalem Gesichtsschädelaufbau. Die Ursache der behinderten Atemfunktion bildete in erster Linie eine funktionelle Verlegung des Cavum nasi durch Septumanomalien oder Schleimhautschwellung, während die Größenentwicklung der Adenoide und Tonsillen von untergeordneter Bedeutung war. Abweichend zu den anderen Dyskinesien trat die viszerale Schluckart deutlich seltener zusammen mit einer Verlegung der oberen Luftwege auf. Die orofazialen Dyskinesien konnten durch Adeno- und/oder Tonsillektomie nicht abgestellt werden. Im Gegensatz zur behinderten Nasenatmung waren für den vertikalen Wachstumstyp, die viszerale Schluckart und das einfache Zungenpressen die schlechteren Tuben- und Mittelohrbefunde charakteristisch.
Summary
In 120 11-year old orthodontic patients the relationship between pathological ENT-findings and orofacial dysfunctions (i. e. incompetent lips, simple tongue-thrust and persistent infantile swallowing pattern) was examined and the results compared to patients with open bites and a vertical growth pattern. According to the rhinological and rhinomanometric results, impaired nasal breathing was more frequently observed in patients with incompetent lips and simple tongue-thrust than in older children with open bites and a vertical growth pattern. Nasal airway inadequacy was primarily due to functional obstruction of the nasal cavity by septal anomalies and congestion of the nasal mucosa, enlarged adenoids and tonsils being of little significance. In contrast to the other dysfunctions, no correlation was found between the persistent infantile swallowing pattern and mode of breathing. Adenoidectomy and tonsillectomy did not eliminate the orofacial dysfunctions. Independent of the parameter impaired nasal breathing, patients with vertical growth patern, abnormal type of swallowing or simple tongue-thrust showed an increased incidence of disturbed tubal and middle ear ventilation.
Résumé
Chez 120 patients orthodontiques de 11 ans, on a examiné les dysfonctions oro-faciales (l'incompétence labiale, la pulsion simple de la langue et la déglutition infantile) au point de vue de leurs anomalies otorhino-laryngologiques. Les résultats ont été comparés avec des patients ayant une béance ainsi qu'avec ceux présentant un type de croisance crânio-faciale à prédominance verticale. Selon les résultats rhinoscopiques et rhinomanométriques, la respiration nasale a été plus fréquemment contrariée chez des patients présentant soit une incompétence labiale, soit une pulsion simple de la langue que les cas présentant une béance et un type crânio-faciale à prédominance verticale. La gêne respiratoire nasale résultait en premier lieu d'un rétrécissement dans les fosses nasales imputable à des déformations de la cloison ou au gonflement de la muqueuse nasale. Au contraire l'hypertrophie adénoïdienne et amygdalienne étaient de moindre importance. La déglutition infantile différait distinctement des autres dysfonctions, telle que celle-ci n'a rien à voir avec un rétrécissement des voies aériennes hautes. Les dysfonctions oro-faciales ne pouvaient pas être éliminées par l'adénoïdectomie et/ou par l'amygdalectomie. Indépendamment de la gêne respiratoire nasale, les patients avec un type de croissance crânio-faciale à prédominance verticale ainsi que la déglutition infantile et la pulsion simple de la langue étaient charactérisés par une dysfonction de la trompe d'Eustache associés à une ventilation insuffisante de l'oreille moyenne.
