Die Reaktion des Parodonts auf kieferorthopädische Kräfte bei diabetischer Stoffwechsellage

Zusammenfassung

Die Reaktion des Parodonts auf kieferorthopädische Kräfte wurde bei der Streptozotocin-diabetischen Ratte (Typ-I-Diabetes) untersucht. Die zur Mesialisierung des ersten Molaren verwendete Kraft betrug 10 N, 20 N und 30 N, die Applikationsdauer drei, sechs, zehn und 14 Tage. Es zeigte sich, daß die diabetische Stoffwechsellage zu charakteristischen Veränderungen im Parodont führt. Im Vordergrund stehen: 1. die stark verzögerte bzw. ausbleibende Knochenneubildung, die auch fluoreszenzmikroskopisch nachweisbar ist, 2. die Unfähigkeit des fraktioniert gebildeten neuen Knochens, Fluorochrome sequentiell zu binden, 3. eine rarefizierende Ossifikation, 4. eine Involution bzw. Schwächung des Ligamentumparodontale, 5. Mikroangiopathien im Bereich der Gingiva. Die diabetesspezifischen Veränderungen im Zahnhalteapparat verstärken sich während einer kieferorthopädischen Zahnbewegung.

Summary

The reaction of the periodontium to orthodontic tooth movement was studied in rats with Streptozotocindiabetes. Forces employed to initiate mesial movement of first molar were 10 N 20 N 30 N and the duration of application was three, six, ten and 14 days. The 14 day-rats were treated with fluorochrome sequential labelling according toRahn. Diabetes mellitus in rats is accompanied by characteristic changes: The most important findings were: 1. a slow osseous regeneration, as could be shown by fluorescentmicroscopy, 2. the new bone ist unable to pick up the labellings in a sequential manner, 3. a weatening of the periodontale ligamentum, 4. microangiopathies in the gingiva area, the specific diabetic changes are more pronounced following orthodontic tooth movement.

Résumé

La réaction du parodonte après une injection de Streptozotocin chez des jeunes rats a provoqué un diabète de Type I. «Diabetes mellitus» provoque une modification très significantive du parodonte après application d'une force orthopédique sur la première molaire au maxillaire. (Force: 10 N, 20 N, 30 N; trois, six, dix, 14 jours). La molaire supérieure est dirigée en direction mésiale avec un appareillage fixe. L'étude histologique montre: 1. une régénération très lente de l'os. 3. une régression du ligament parodontal. 4. des microangiopathies au niveau de la gencive.
