Zusammenfassung
Die Dysostosis cleidocranialis ist im Bereich der Zähne und Kiefer durch die Hyperodontie, eine Dentitio tarda, Zahnretentionen und eventuell Follikularzysten gekennzeichnet. Ein aufgestelltes Behandlungskonzept erlaubt in zwei Etappen 1. etwa bei Beginn der verspäteten zweiten Dentition (Dentitio tarda) die operative Entfernung der überzähligen Zahnkeime und 2. entsprechend der verspäteten morphologischen Entwicklung (Dentitio tarda) die operative Freilegung der retinierten Zähne der zweiten Dentition. Der erste operative Eingriff ermöglicht und veranlaßt die hindernisfreie morphologische und besonders vertikale Entwicklung der Zähne der zweiten Dentition, der zweite wird notwendig, wenn die morphologische Entwicklung der Zähne der zweiten Dentition abläuft, nicht aber ihre Vertikalentwicklung. Die operative Freilegung geschieht nach einer selbst entwickelten Operationsmethode [13, 18]. Gleichzeitig mit der freien spontanen Vertikalentwicklung der freigelegten Zähne erfolgt die kieferorthopädische Behandlung der außerdem vorliegenden Dysgnathie.
Summary
Dysostosis cleidocranialis concerns both teeth and jaw and is characterized by supernumerary teeth, Dentitio tarda, tooth-impaction and eventually follicular cysts. The concept of treatment can be divided in two stages: 1. At the beginning of the delayed secondary dentition (Dentitio tarda) the operative removal of the supernumerary toothgerms and 2. corresponding to the belated morphological development (Dentitio tarda) the operative exposure of the impacted teeth of the secondary dentition. The first operation facilitates a morphological development and especially a vertical drift without any obstacles. The second becomes necessary when only the morphological development, but not the vertical drift of the teeth of the secondary dentition takes place. The operative exposure is done using a self-developed technique [13, 18]. At the same time as the exposed teeth undergo a spontaneous vertical drift, orthopedic treatment for anomaly is started.
Résumé
La dysostose cléidocranienne se caractérise au niveau des dents et des maxillaires, par des dents surnuméraires, des éruptions tardives, des inclusions dentaires et éventuellement des kystes folliculaires. Le plan de traitement est divisé en deux étapes: 1. Au début de la deuxième période de la dentition retardée (Dentitio tarda) les germes surnuméraires sont enlevés opérativement et 2. correspondant au développement morphologique tardif (Dentitio tarda), les dents incluses de la deuxième denture sont dégagées opérativement. L'opération primaire rend possible aux dents de la deuxième denture d'avoir un développement morphologique et surtout un développement vertical sans obstacles. Une deuxième opération sera nécessaire au moment où le développement morphologique et non le développement vertical des dents de la deuxième denture se manifeste. Le dégagement opératoire se fait selon une technique opératoire développée par nous-même [13, 18]. Au moment du développement vertical spontané des dents dégagées, on commence la thérapie orthopédique de cette dysgnathie.
