Zusammenfassung
In der Untersuchung wird der Frage nachgegangen, ob durch kieferorthopädische Maßnahmen mit herausnehmbaren oder festsitzenden Apparaturen eine Verschlechterung der marginalen parodontalen Verhältnisse hervorgerufen und somit die Entwicklung parodontaler Erkrankungen begünstigt wird. Der parodontale Zustand der Probanden wurde anhand der Parameter PLI, PBI, SFFR und Sulcustiefe bestimmt.
Die Ergebnisse der Untersuchung lassen sich wie folgt zusammenfassen: 1. Bei Trägern herausnehmbarer kieferorthopädischer Apparaturen lassen sich keine deutlichen Unterschiede zur Kontrollgruppe unbehandelter Probanden feststellen. 2. Träger feststizender kieferorthopädischer Apparaturen haben im Vergleich zur Kontrollgruppe bei gleichem PLI fast durchwegs ungünstigere Werte für den Papillenblutungs-Index und die „Sulcus-Fluid-Flow-Rate” zu verzeichnen. 3. Der Einfluß des Alters der Probanden auf die Untersuchungsergebnisse läßt sich anhand dieser Studie nicht beurteilen. 4. Auffallendstes Ergebnis der Untersuchung ist, daß die Mundhygiene nahezu aller untersuchten Gruppen nicht ausreichend ist und somit keine gesunden parodontalen Verhältnisse vorliegen. Zahnbewegungen sollten jedoch grundsätzlich nur am gesunden Parodont durchgeführt werden.
Summary
The study was designed to evaluate the adverse influence of removable or fixed orthodontic appliances on the periodontal status of patients, thus predisposing to periodontal disease. PLI (Plaque Index), PBI (Papillary Bleeding Index), SFFR (Sulcus-Fluid-Flow-Rate) and probing depth were the parameters used to evaluate the periodontal health of the patients in the study group.
The results of the investigation were: 1. There was no significant difference between wearers of removable appliances and the untreated control group. 2. Wearers of fixed orthodontic appliances showed increased values of the Papillary Bleeding Index (PBI) and the Sulcus-Fluid-Flow-Rate (SFFR) in contrast to the control group. 3. The small patient population did not allow for definite conclusions to be made on the relationship between the age of the patients and the results of the investigation. 4. The most striking observation of the study was a decreased ability to maintain oral hygiene with a consequent decline in periodontal health. Orthodontic movements however should be carried out basically on teeth with a healthy periodontium.
Résumé
L'étude analyse la question de savoir si par des mesures orthopédiques prévoyant, ou bien des appareils amovibles, ou bien des appareils fixes, la situation parodontale marginale a subi une détérioration et si par ceci, l'évolution des maladies parodontales a été favorisée. La situation parodontale des sujets a été déterminée sur la base des paramètres suivants: le PLI (indice de plaque), le PBI (indice de saignement papillaire), le SFFR (taux d'écoulement du liquide dans le cul-de-sac) ainsi que de la profondeur des culs-desac gingivo-dentaires.
Les résultats de l'étude peuvent être résumés comme suit: 1. Entre les porteurs d'appareils orthopédiques et le groupe témoin des sujets sans appareils, on n'a pas constaté des différences sensibles. 2. Lorsque l'indice de plaque est identique, les porteurs d'appareils orthopédiques ont, par rapport au groupe témoin, presque toujours des valeurs moins favorables, aussi bien d'indice de saignement papillaire que de taux d'écoulement du liquide dans le cul-de-sac gingival. 3. Cette étude ne permet pas de déterminer l'influence de l'âge des sujets sur les résultats. 4. Le résultat le plus éclatant de l'étude est le fait que l'hygiène dentaire de pratiquement tous les groupes est insuffisante et qu'ainsi la situation parodontale n'est pas saine. Les mouvements dentaires, cependant, ne peuvent en principe être effectués que sur un parodonte sain.
