CRP-Reaktion und Blutkörperchensenkungsgeschwindigkeit bei Lungentuberkulose

Zusammenfassung

Nach allgemeinen Ausführungen über die Geschichte und die derzeitige Anwendung der CRP-Reaktion werden die Ergebnisse an 1000 Fällen von behandlungsbedürftiger Tuberkulose besprochen. Es ergibt sich, daß der Ausfall weitgehend von dem Stadium der Behandlung abhängt. Die CRP-Reaktion ist nicht geeignet, die BKS zu ersetzen. Wohl ist sie aber imstande, die BKS und andere unspezifische Reaktionen zu ergänzen. Als Routineuntersuchung kommt sie wohl nicht in Frage, die Anwendung ist aber dann zu empfehlen, wenn zwischen BKS und klinischem Bild keine Übereinstimmung besteht. So kann bei frischen Fällen eine niedrige BKS durch eine positive CRP-Reaktion entlarvt werden. Vor allem kann auf diese Weise geprüft werden, ob eine verordnete Behandlung auch wirklich durchgeführt wurde. Eine positive CRP-Reaktion bei durchgeführter Tuberkulosebehandlung weist in den meisten Fällen auf eine Zweitkrankheit hin oder eine Streuung, die so frisch ist, daß sie durch die BKS noch nicht zum Ausdruck kommt.

Summary

The results of 1000 cases of treatment requiring tuberculosis were discussed after some general statements concerning the history and the current utilization of the CRP-reaction. It was seen that the reaction was dependent to a great extent upon the specific stage of treatment. The CRP-reaction is not qualified to replace the ESR. It is, however, well qualified to supplement the ESR and other nonspecific reactions. It is also out of the question to utilize it as a routine examination. It should rather be employed in those instances when a satisfactory conformity is missed between the ESR and the clinical situation. It is thereby possible to expose fresh cases, with a low ESR, through a positive CRP-reaction. This reaction lends itself expecially to the examination as to whether a specified type of therapy may be truly satisfactorily completed. A positive CRP-reaction in the presence of a completed therapeutic program indicates in most cases that a second disease is present, or that a tuberculous metastases has taken place which is so fresh that it is as yet not recognizable by a change in the ESR.
