Abstract
Experiences in 21 patients with alveolar cell carcinoma of the lung admitted to a pneumophthisiology department between 1949 and 1972 are reported. Diagnosis was established by autopsy in 3 cases, by histological examination of material from lung resection in 10 cases, by lingula biopsy in 1 case and by cytological studies in 7 cases. Resection proved possible in 10 cases; 5 of these patients have remained free of disease, 3 of whom are living after 1, 8 and 13 years while 2 of them died 3 and 8 years postoperatively. Of the other 11 patients only 2 are alive, 4 and 8 months after diagnosis. Individual case histories are used to illustrate different radiographic appearances and clinical courses of the disease, and the difficulty of establishing diagnosis by cytology only is discussed. Based on reviews of the literature, frequency among bronchogenic carcinoma, distribution by sex and age, smoking habits and therapy are discussed and compared to those for bronchogenic carcinoma. Finally stress is laid on the fact that diagnosis of alveolar cell carcinoma should be followed by the same measures as applied in other histological types of bronchogenic carcinoma. If possible every patient should be submitted to surgery without delay.
Zusammenfassung
Es wird über Erfahrungen bei 21 Alveolarzellcarcinomen der Lungen berichtet, die zwischen 1949 und 1972 an einer Lungenabteilung diagnostiziert werden konnten. Die Diagnose wurde 3mal durch Obduktion, 10mal durch Untersuchung des Resektionspräparates, 1mal durch Lingulabiopsie und 7mal durch Exfoliativcytologie gestellt. Bei 10 Pat. konnte eine Resektion durchgeführt werden: 5 blieben rezidivfrei, davon leben 3 Pat. 1, 8 und 13 Jahre; 2 verstarben 3 bzw. 8 Jahre postoperativ. Von den 11 nicht operierten leben nur noch 2 Pat. 4 bzw. 8 Monate nach der Diagnosestellung. Anhand einzelner Fallbeispiele werden verschiedene röntgenologische Erscheinungsbilder und Verlaufsformen geschildert und auf die Problematik der alleinigen cytologischen Diagnosestellung Alveolarzellcarcinom hingewiesen. Aufgrund von Literaturübersichten wird zur Häufigkeit unter den Bronchuscarcinomen, zur Geschlechts- und Altersverteilung, dem Nicotinkonsum und der Therapie Stellung genommen und Vergleiche zum Bronchuscarcinom angestellt.
