Depression and psychosocial adjustment in adolescent anorexia nervosa. A controlled 3-year follow-up study

Abstract

This study investigated depressive psychopathology and psychosocial functioning in adolescent anorexia nervosa patients at a three year follow-up in comparison to healthy age-matched controls. Three standardized rating instruments (HRSD, SDS, BDI) were used for assessing depression and the Morgan-Russell scales for defining the outcome of the eating disorder. Our findings suggest 1) a highly positive correlation between eating disorder and depressive psychopathology; 2) recovered anorectics score higher on depression scales than healthy controls; 3) recovered and unrecovered anorectics are similar to age-matched controls in terms of occupational adjustment, social contacts and dependency on family, but differ significantly in psychosexual functioning.

Résumé

Cette étude étudie la psychopathologie dépressive et le fonctionnement psychosocial de patientes anorexiques mentales adolescentes avec un recul de 3 ans en les compararant à un groupe contrôle sain du même âge. 3 instruments d'évaluation standardisés (HRSD, SDS, BDI) furent utilisés pour évaluer la dépression ainsi que l'échelle de Morgan-Russell (Morgan et Russell, 1975; Morgan et Hayward, 1988) pour définir le devenir du trouble alimentaire. Nos résultats suggèrent: 1) une corrélation positive élevée entre le trouble alimentaire et la psychopathologie dépressive; 2) un score d'anorexiques guéries plus élevé à l'échelle de dépression qu'au groupe contrôle sain; 3) les anorexiques guéries et non guéries sont semblables au groupe contrôle en terme d'adaptation occupationnelle, de contacts sociaux et de dépendance à la famille, mais diffèrent significativement quant au fonctionnement psychosexuel.

Zusammenfassung

Die vorliegende Studie untersucht die depressive Symptomatik sowie die psychosoziale Adaptation jugendlicher anorektischer Patienten 3 Jahre nach Entlassung im Vergleich zu gleichaltrigen gesunden Kontrollpersonen. Untersuchungsinstrumente sind drei standardisierte Verfahren zur Erfassung der Depressivität. Zur Beurteilung des Heilungserfolges bei der Eßstörung werden die Kriterien von Morgan & Russell 1975 (General und Average Outcome Score) verwandt. Unsere Ergebnisse beinhalten: 1) die depressive Symptomatik zum Katamnesezeitpunkt hängt signifikant von der Schwere der Eßstörung ab; 2) selbst anorektische Patienten mit einem gutem Heilungserfolg weisen höhere Werte auf den Depressionsskalen auf als ihre gesunden Kontrollen; 3) Patienten mit gutem und schlechtem Heilungserfolg unterscheiden sich nicht von gesunden Gleichaltrigen in Bezug auf berufliche Anpassung, soziale Kontakte und Abhängigkeit von der Familie, während in den Bereichen Partnerschaft und Sexualität auch bei den Patienten mit gutem Heilungserfolg keine Angleichung an die Kontrollgruppe erfolgt ist.
