Zusammenfassung
Unter 379 natürlich vorkommenden Staphylokokkenstämmen wurden 36 Stämme mit der sogenannten „inhärenten Penicillinresistenz“ (IPR) gefunden. Alle 36 Stämme gehören zur Phaggruppe III und zeigen Mehrfachresistenz. Nach Phagmuster und Resistenzbild lassen sie sich in 14 epidemiologisch unterschiedliche Typen einteilen.
Jeder Stamm zeigt, durch genetische Instabilität bedingt, heterogenes Resistenzverhalten. Oft tolerieren alle Zellen einer Population von 108 Keimen eine nur kleine Antibioticumkonzentration — diese entspricht der sogenannten Grundresistenz -, und nur Minoritäten der Population sind gegen hohe Penicillinkonzentrationen resistent. Seltener werden auch Stämme mit hoher Grundresistenz gefunden.
Vergleichende Virulenzuntersuchungen an der Maus mit 3 Methoden belegen, daß die Virulenz eines Stammes abnimmt, je höher seine IPR ansteigt, daß aber auch Stämme mit hoher Grundresistenz und erheblicher Virulenz vorkommen können. Über die klinische Bedeutung dieser Stämme wird diskutiert.
