Zusammenfassung
Der Vorgang der Gasdiffusion zwischen Alveolen und Lungencapillarblut kann mit Sauerstoff oder kohlemmonoxyd gemessen werden. Im Gegensatz zu der einfachen physikalischen Definition der pulmonalen Diffusionskapazität (DFO2 oder DFCO) sind die Einzelheiten der Messung und die Interpretation der Ergebnisse kompliziert. Weder die O2-Verfahren (Bohr, Riley undCournand, Bartels, Thews) noch die gebräuchlicheren CO-Methoden (Atemanhaltetechnik nachKrogh, modifiziert durchOgilvie undForster; steady-state-Methoden nachfilley u. Mitarb.,Bates u. Mitarb.,Marshall; Rückatmungsmethode nachLewis u. Mitarb.) können beanspruchen, die wahren physikalischen Diffusionseigenschaften der alveolo-capillären Membran, den Diffusionsprozeß im Capillarblut und, schließlich, die Kinetik von Hämoglobin mit O2 oder CO zu messen. für klinische Zwecke können verschiedene indirekte Methoden (Sauerstoffsättigung bei Ruhe und unter Arbeit) nützliche Informationen über den „Gasübertritt“ zwischen Lunge und arteriellem Blutgeben. Zur Zeit kann keine Methode als einfaches Routineverfahren im klinischen Labor empfohlen werden. Wofür man sich entscheidet, hängt von zahlreichen Faktoren ab, beispielsweise den Untersuchtne, der Ausrüstung und dem technischen Hilfspersonal. Das bisher ungelöste Hauptproblem ist die Beurteilung der ungleichmäßigen Ventilation und der Ventilations-Perfusions-Verhältnisse innerhalb der Lunge.
Summary
The process of diffusion between the alveoli and pulmonary capillary blood can be measured with oxygen or carbon monoxide. In contrast to the simple physical definition of pulmonary diffusing capacity (DFO2 or DFCO) the details of its measurement and the interpretation of the results are complex.
Neither the O2-Techniques (Bohr, Riley andCournand, Bartels, Thews) nor the widely used CO-methods (Krogh's single-breath method, modified byOgilvie andForster; steady-state methods byFilley et al.,Bates et al.,Marshall; rebreathing method byLewis et al.) can be claimed to measure the true physical diffusion properties of the alveolo-capillary membrane, the diffusion process within the capillary blood and, finally, the kinetics of hemoglobin with O2 or CO. For clinical purposes several indirect methods (oxygen saturation at rest and on exercise) may give useful information on the overall “gas transfer” between lung and arterial blood.
At present no method can be recommended as simple and reliable procedure in the clinical laboratory. The choice depends on numerous factors, such as the subjects to be studied, the equipment and the technical help. The main problem, so far unsolved, seems to be the evaluation of uneven ventilation and of ventilation-perfusion ratios within the lung.
