Zusammenfassung

Ohne Anspruch auf Vollständigkeit war an Hand ausgewählter praktischer Gesichtspunkte zu zeigen, daß auch die Analyse der Druck-Volumen-Relationen der Lunge noch einige atemmechanische Probleme, besonders unter pathologischen Bedingungen aufwirft. Schon diebekannten Fehlerquellen geben Anlaß, das Verfahren als semiquantitativ zu bewerten. Durch den Ausbau der Körperplethysmographie erfährt die Methode zwar eine Einengung im Anwendungsbereich der obstruktiven Ventilationsstörung. Sie bleibt aber vorerst die Methode der Wahl bei der atemmechanischen Analyse der restriktiven Parenchymerkrankungen und anderer Störungsmechanismen, bei denen nicht der Alveolardruck, sondern der transpulmonale Druck den entscheidenden Indikator darstellt.

Summary

It was demonstrated, that the analysis of the pressure - volume relationship of the lung evaluated with means of the esophageal pressure method, presents still some problems in method and pathophysiology respectively.

The well known errors, which may occur with this method make it a semiquantitative one with respect to the determination of the static pressure-volume relationship. The development and availability of the bodyplethysmography decreases the range of applicability without doubt, especially in obstructive pulmonary diseases. However, the determination of the esophageal pressure-volume relationship still remains the method of joice in the analysis of the mechanical properties of the lung in restrictive disturbances of ventilation in parenchymal diseases, in which not the alveolar (non-elastic) pressure, but the elastic component of the transpulmonary pressure represents the determining factor.
