Zusammenfassung
Wegen definitorischer und methodischer Schwierigkeiten wird vorgeschlagen, im Rahmen der Pathophysiologie der Lunge auf die Bezeichnung „Alveolarluft“ zu verzichten und in Anlehnung an gebräuchliche Methoden der Lungenfunktionsdiagnostik besser von einer Analyse der Exspirationsluft unter Angabe der zeitlichen Beziehung zur Exspirationsphase zu sprechen.
Die alleinige Bestimmung der endexspiratorischen Partialdrucke von O2 und CO2 kann schon eine alveolare Hypo- oder Hyperventilation aufdecken.
Der Informationsgehalt kontinuierlicher Analysen der Exspirationsluft ist jedoch wesentlich größer. Für die Beurteilung der ventilatorischen Luftverteilung, der Perfusion und auch der Verteilung der Transfer-Eigenschaften verschiedener Gase ist diese schnell durchführbare und den Probanden nicht belästigende Methode unerläßlich.
Summary
Because of difficulties in method and definition it is suggested to avoid term “alveolar gas” in the pathophysiology of lung. Due to usual methods in the determination of lung function it may be better to use the term of expiratory gas in relation to the phase of expiration.
The only determination of final-expiratory partial pressure of O2 and CO2 yields the diagnosis of alveolar hypoventilation or hyperventilation.
However, the information of continuous analysis of expiratory gas is more interesting. For the assessment of distribution of ventilation, perfusion and the transfer properties of different gases this method is a practicable way. The examination is fast and not molesting the patients.
