Zusammenfassung
Die Dynamik und Kontraktilität der hypertrophierten linksventrikulären Muskulatur wurde bei 16 Patienten mit Aortenstenosen unterschiedlichen Schweregrades untersucht und mit 4 Patienten ohne Aortenvitium und Hypertonie verglichen. Die Patienten wurden je nach Schweregrad des Leidens in vier Gruppen unterteilt.
Neben einem deutlichen Anstieg des systolischen Gradienten zwischen unblutig gemessenem Brachialarteriendruck und linksventrikulärem Druck ließ sich auch eine allmähliche Erhöhung des enddiastolischen Volumenindex bei Zunahme des Schweregrades der Aortenstenose erkennen. Eine Vergrößerung des Schlagvolumenindex konnte erst bei den Aortenstenosen IV festgestellt werden.
Die Muskelmasse und die enddiastolische Muskelwandstärke über dem linken Ventrikel zeigten wie die Parameter ihrer Kontraktionskraft eine klare Abhängigkeit vom Schweregrad der Aortenstenose. Das ließ sich in einer stufenweisen Zunahme des Kontraktilitätsindex, der maximalen Druckanstiegs- und der maximalen Verkürzungsgeschwindigkeit bis einschließlich dem III. Schweregrad der Aortenstenose nachweisen. Das Zeitintervall vom a-Wellenende bis zu max dP/dt (time to peak dP/dt) verkürzte sich entsprechend. Bei Aortenstenosen III konnten bei jungen Patienten maximal erhöhte Kontraktilitätsindices gefunden werden.
Beim IV. Schweregrad des Leidens mußte jedoch ein deutliches Nachlassen der Kontraktilität mit einem Absinken aller Parameter und einer Verlängerung der time to peak dP/dt registriert werden. Die Werte dieser Patientengruppe lagen dabei unter denen der Kontrollgruppe ohne Aortenvitium oder Hypertonie.
Summary
Left ventricular dynamics and contractility of the hypertrophied muscles were studied in 16 patients suffering in valvular aortic stenosis of a different degree and compared with those of 4 patients with assumed normal myocardial function. The patients were subdivided in 4 groups according to the severity of their disease.
In addition to a significant augmentation of the systolic gradient between aortic and left ventricular pressure small rises of the enddiastolic volume (EDVI) were seen, when the severity of the disease increased. The stroke volume (SVI) only showed an increase in the patient group with aortic stenosis IV.
According to the signs of contractility left ventricular muscle mass and enddiastolic wall thickness demonstrated an exact relation to the degree of severity of the disease.
Max dP/dt, Vmax and the index of contractility showed a significant rise in groups with higher degrees of severity including the group with aortic stenosis III. In young patients with aortic stenosis III indexes of contractility with a maximum of augmentation were observed. The time to peak dP/dt thereby was shortened extensively.
When the disease has reached the fourth degree of severity however the values of myocardial function significantly shifted downward and decreased those of the control group without aortic valve disease or arterial hypertension.
