Zusammenfassung
Die linksventrikuläre Herzmuskeldynamik wurde bei 16 Patienten mit einer Aorteninsuffizienz unterschiedlichen Schweregrades untersucht und mit 4 Patienten ohne Aortenvitium oder Hypertonie verglichen. Die Patienten wurden dazu in vier Gruppen je nach Schweregrad des Leidens unterteilt. Neben einem Anstieg der Blutdruckamplitude konnte auch eine deutliche Erhöhung des totalen Schlagvolumens, des Regurgitationsvolumens und des enddiastolischen Volumens mit zunehmendem Schweregrad des Leidens beobachtet werden.
Die Muskelmasse und die enddiastolische Muskelwandstärke über dem linken Ventrikel jedoch zeigte keine proportionale Abhängigkeit vom Schweregrad der Aorteninsuffizienz.
Die Parameter der Muskelkontraktilität wiederum zeigten eine strenge Parallelität, die bis einschließlich dem III. Schweregrad der Aorteninsuffizienz in einer stufenweisen Zunahme des Kontraktilitätsindex, der maximalen Druckanstiegs- und Verkürzungsgeschwindigkeit zum Ausdruck kam. Das Zeitintervall von a-Wellenende zu max dP/dt (time to peak dP/dt) verkürzte sich entsprechend. Bei den Aorteninsuffizienzen IV. Schweregrades mußte jedoch ein deutlicher Abfall der Herzmuskelkontraktilitätsparameter bei weiterer Zunahme der Volumina registriert werden.
Summary
Left ventricular dynamics and contractility were studied in 16 patients suffering in valvular aortic insufficiency of different degree. These functions were compared with those of 4 patients without aortic valve disease or arterial hypertension. The patients were subdivided in 4 groups according to the severity of their disease.
In addition to an increase of the pressure-amplitude significant rises of the total cardiac output, of the backward-volume and of the enddiastolic volume were observed in the patient groups with higher degrees of the severity.
Left ventricular muscle mass and enddiastolic wall thickness however showed no exact relation by it.
The signs of left ventricular contractility corresponded exactly to the severity of the aortic insufficiency including its third degree.
Max dP/dt, Vmax and the index of contractility increased in conformity with the degrees of severity of the disease and the time to peak dP/dt thereby was shortened.
When the fourth degree of severity was reached a significant decrease of the signs of contractility was recorded whereas the heartvolumes further shifted upward.
