Zusammenfassung
Tierexperimentell sollte ein Sterilisationsmodell entwickelt werden, bei dem die nachfolgenden Eileiterveränderungen möglichst gering und die Bedingungen für eine spätere Anastomosierung möglichst gut sind.
Dazu wurde in einer 1. Gruppe bei 17 Kaninchen eine Ligatur des Tubenisthmus vorgenommen, in einer 2. Gruppe wurde bei 18 Kaninchen die Tubenampulle im mittleren Bereich unterbunden.
Eine, 4 und 12 Wochen nach Anlage der Ligaturen durchgeführte licht-, raster- und transmissionselektronenmikroskopische Untersuchungen ergaben folgende Veränderungen: Nach isthmischer Ligatur trat eine Einengung der Lichtung mit Abflachung der Schleimhaut und Differenzierungsverlust des Schleimhautepithels ein, verbunden mit einer leichten Vernarbung der Tubenwand. Diese Veränderungen waren auf den Ligaturbereich beschränkt.
Nach ampullärer Ligatur entwickelte sich bei 27 der 35 untersuchten Eileiter eine Hydrosalpinx zwischen der Ligatur und dem ampullär-isthmischen Übergangsbereich. Eine Abflachung der Schleimhaut mit Differenzierungsverlust des Schleimhautepithels war nicht nur im Ligaturbereich sondern auch in den erweiterten Ampullenabschnitten ausgeprägt.
Im Vergleich zur ampullären Ligatur bietet die isthmische den Vorteil, daß hier die Eileiterveränderungen auf ein kurzes Segment beschränkt sind. Nach Resektion dieses Segmentes und nachfolgender Anastomosierung dürften gute Voraussetzungen für eine Refertilisierung bestehen.
Summary
An animal model of tubal sterilization would be valuable to examine the problem of tubal wall damages and to study the criteria for optimal anastomosis.
In a first experimental group of 17 rabbits the isthmic segments of Fallopian tubes were liagted. In a second group of 18 rabbits the ampullary segments of oviducts were ligated.
One, 4, and 12 weeks after ligation examinations by light microscopy, scanning and transmission electron microscopy showed a number of alterations: After isthmic ligature there was a constriction of the tubal lumen with flattening and loss of differentiation of the mucosal epithelium and slight scarring of the tubal wall under the ligature but not elsewhere.
After ampullary ligature a hydrosalpinx developed in 27 of 35 oviducts between the ligature and the ampullary-isthmic junction. The mucosa became flattened and cell differentiation was impaired not only in the area of the ligature but also in the dilated ampullary segments.
Thus, we would advise against sterilization by ligature of the tubal ampulla because ligation of the isthmic segments provides much better conditions for restoration of fertility by anastomosis of ligated Fallopian tubes.
