Zusammenfassung
Vor einigen Jahren war ein Phantomschmerzleidender durch Unterschneidung der Rindenfelder 9 und 10 beschwerdefrei gemacht worden. Nach einer Gehirnerschütterung traten zugleich mit den typischen Hirntraumazeichen die alten Beschwerden erneut auf und bildeten sich zusammen mit diesen im Zuge der Heilung wieder zurück. Zur Erklärung dieser eigenartigen Erscheinung wird angenommen, daß durch Commotionierung der Substantia reticularis des Mittel-Zwischenhirns die schmerzhaften Phantomimpulse nach und über thalamo-kortikale und kortiko-thalamische Bahnen fehlgeleitet wurden, auf denen sie die ihnen physiologische, jedoch durch die Leukotomie blockierte Strecke umgehen konnten. Unter (anticholinergischer) Behandlung wurde dann die retikuläre „Weiche“ offenbar auf das alte, an der Unterschneidungsstelle blind endigende Gleis zurückgestellt.
Summary
Some years ago a patient with phantom pain was freed of his complaint by cortical undercutting of the areas 9 and 10. After a brain concussion the old troubles reappeared together with the typical symptom of cerebral trauma and disappeared together with the latter in the course of recovery. As an explanation for this strange phenomenon it is believed that in consequence of the concussion of the meso-diencephalic reticular substance, the painful phantom impulse was erroneously switched to and over the thalamo-cortical and corticothalamic pathways, hereby avoiding the physiological pathways blocked by the leucotomy. Under anticholinergic treatment, the reticular switch was evidently readjusted to the old tract ending dead at the point of the cortical undercutting.
Résumé
Il y a quelques années un malade souffrant d'une douleur fantôme, fut guéri de ses troubles par la section sous-corticale des aires 9 et 10. Après un choc crânien, les anciens troubles se manifestèrent de nouveau, associés aux symptômes typiques du trauma cérébral, mais au cours de la guérison toute la Symptomatologie disparaissait. Comme explication de cet étrange phénomène, on suppose que, par suite de la commotion de la substance réticulaire du méso-diencéphale, les impulsions douloureuses ont été erronément conduites vers et sur les voies thalamiques-corticales et, à travers ces voies, les voies physiologiques, bloquées par le leucotomie, pouvaient être évitées. Au moyen du traitement anticholinergique, le Weiche» réticulaire fut évidemment reporté sur les vieilles voies qui terminent sans issue en correspondance du lieu de la section.

Qualche anno prima, un malato affetto da dolore fantasma, fu liberato dai disturbi per mezzo della sezione sottocorticale dell'area 9 e 10. Dopo un trauma cranico, ritornarono, associati ai tipici sintomi del trauma cerebrale, i vecchi disturbi, che insieme ai nuovi sparivano nel corso della guarigione. Come spiegazione di questo strano fenomeno si suppone che a causa della commozione della substantia reticularis del meso-diencefalo, gli impulsi dolorosi siano stati condotti erroneamente verso e sopra le vie talamico-corticali e attraverso queste vie potevano essere evitale le vie fisiologiche, bloccate dalla leucotomia. Per mezzo del trattamento anticolinergico il „Weiche“ reticolare fu evidentemente riportato sulle vecchie vie che terminano a fondo cieco in corrispondenza del luogo della sezione.
Resumen
Algunos años antes, un enfermo que padecia de dolor fantasma, fué liberado de sus molestias por medio de la sección sotocortical de las areas 9 y 10. Después de un trauma craneal, volvieron, asociados a los tipicos sintomas de el trauma cerebral, las antiguas molestias, que juntas a las nuevas, desaparecían en el curso de la curación. Come esplicación de este extraño fenómeno, se supone que, por la comoción de la substancia reticularis de el mesodiencefalo, las impulsiones dolorosas han sido conducidas por error hacia y sobra los senderos talamicos-corticales, y, por estos senderos podian ser evitados los senderos fisiológicos, cerrados por la leucotomia. Por medio de la cura anticolinergica, el cambio reticular fué evidentemente puesto otra vez sobra los senderos de antes che terminan a fondo ciego en correspondencia de el lugar de sección.
