Abstract
Early T1 and small T2 low rectal cancers may be controlled by endocavitary irradiation using the 50 kV Philips machine. The ambulatory treatment performed in the out-patient department consists of 4 applications within 6 weeks. Iridium-192 implant performed under local anesthesia is useful in many cases to give a booster dose to the tumor bed. In a series of 312 patients followed for >5 years, the rates of local and nodal failure were 4.5% and 3.8%, respectively. The rate of death from cancer was 7.7%. After local excision endocavitary irradiation may be used as adjuvant therapy but it is safer to combine external beam and endocavitary irradiation. In the particular case of very poor risk patients with T2 or T3 tumors of the lower third of the rectum, a short course of external beam irradiation (30 Gy within 12 days) followed 2 months later by endocavitary irradiation may be a reliable procedure to prevent permanent colostomy in cases selected according to the patient's condition and the features of the residual disease. Of 67 patients followed for >5 years, 3 patients died of distant metastasis and 5 patients died of local failures. These data, based on close collaboration with surgeons, suggest a reappraisal of the role of radiation therapy in the conservative management of rectal cancer.
Résumé
Il est démontré qu'au stade de début l'adénocarcinome du rectum ne comporte qu'un taux très faible de risque ganglionnaire et peut, dans certaines conditions être traité de manière locale conservatrice. L'irradiation intracavitaire du cancer du rectum se fonde sur l'utilisation de l'appareil de Contact Philips 50 kV et sur l'implantation d'Iridium-192. Elle a trois sortes d'indications : 1/ Employée seule et appliquée aux cancers T1 ou petits T2 sélectionnés sur une série de 312 patients suivis plus de 5 ans, le taux de décès par cancer est de 7.7%. Sur 26 éches locaux ou ganglionnaires 7 ont pû être sauvés par chirurgie. 2/ Elle peut être utile comme complément d'une excision locale. Mais il est préférable de la combiner avec une courte irradiation externe. 3/ Dans le cas particulier de sujets à haut risque opératoire, porteurs de cancer du bas rectum justiciable de l'amputation, l'association d'une irradiation externe (30 Gy en 13 joure) cuivie doux mois plus tard d'irradiation intracavitaire, permet d'éviter bien souvent la colostomie définitive. Sur 67 patients suivis plus de 5 ans (age moyen : 74 ans) le taux de décès par cancer a été de 11.9%. L'irradiation intracavitaire a une place précise et irremplaçable dans le traitement de certains cancers du rectum.

Se ha demostado que el adenocarcinoma rectal en su fase temprana exhibe una baja incidencia de invasión linfática y que métodos alternos a la resección mayor tales como resección local, electrocoagulación o radioterapia primaria pueden ofrecer, en ciertas circunstancias, una posibilidad igual de curación al tiempo que preservan la integridad de la función rectal. Actualmente el adenocarcinoma rectal ya no es considerado como un tumor radioresistente. Los tumores tempranos T1 y los tumores pequeños T2 del recto distal pueden ser controlados mediante irradiación intracavitaria utilizando una máquina Phillips de 50 kV. El tratamiento ambulatorio, que se realiza en forma ambulatoria, consiste en 4 aplicaciones en el curso de 6 semanas. El implante de iridio-192 bajo anestesia local es de utilidad en muchos casos como dosis de refuerzo (“booster”) sobre el lecho tumoral. En una serie de 312 pacientes seguidos por más de 5 años las tasas de falla local y ganglionar fueron 4.5% y 3.8% respectivamente. La tasa de mortalidad por cáncer fue de 7.7%. Después de la resección local se puede utilizar irradicación intracavitaria como terapia adyuvante, pero es más seguro combinar la irradiación externa con la irradiación intracavitaria. En el caso particular de pacientes de muy alto riesgo con tumores T2 o T3 del tercio distal del recto se puede utilizar un régimen corto de irradiación externa (30 Gy en 12 días) seguido 2 meses más tarde de irradiación intracavitaria como un prodecimiento confiable para evitar una colostomía permanente en casos seleccionados de acuerdo a la condición del paciente y las características de la enfermedad residual. De 67 pacientes seguidos por más de 5 años, 3 murieron por metástasis distantes y 5 por falla local. Estos datos, que emanan de una conducta de estrecha colaboración con el cirujano, sugieren que se debe emprender una reevaluación del papel de la radioterapia en el manejo conservador del cáncer rectal.
