Abstract
The authors discuss in detail the etiology, pathogenesis, pathological-anatomical, clinical and radiological picture of diffuse endoalveolar pulmolithiasis. They maintain that innate anomalies in the metabolism of the alveolar cells and exogenous noxious agents give rise to increased production of superficial active substance (surfactant) of the lung alveoli. Being a physiological product of the lung alveoli, this substance does not cause reactive inflammatory changes. In the course of time, degenerated elements of the superficial active substance form a mucoproteinous basis (matrix) upon which calcium salts are deposited.
The authors report 11 cases of this rare disease. They admit that subjective complaints are missing in initial and non-complicated pulmolithiasis because microlites (microstones) lie freely in the alveoli and do not bring about the appearance of an alveolar-capillary block. Some X-ray peculiarities in the observed cases have been mentioned: retouching of the heart and diaphragm contours in initial cases, similar qualitative characteristics of X-ray and morphological changes in the family appearance of the disease, and the appearance of the so-called “pearl necklaces” in the X-ray picture of the lungs during the advanced stage of the disease. The authors propose a new X-ray classification of pulmolithiasis, and maintain that the disease yields pathognomonic X-ray images which offer a reliable diagnosis without the performance of diagnostic thoracthomy or a biopsy investigation of the lung.
Zusammenfassung
Die Autoren vermitteln einen ausführlichen Überblick über Ätiologie, Pathogenese, pathologisch-anatomisches, klinisches und röntgenologisches Bild der Pulmolithiasis endoalveolaris diffusa. Es wird die Auffassung geäußert, daß es infolge einer angeborenen Anomalie des Alveolarzellstoffwechsels zu einer vermchrten Produktion des Oberflächenfilms (Surfactant) der Lungenalveolen kommt. Diese Substanz verursacht keine reaktive Entzündung, weil es sich um ein physiologisches Produkt der Lungenalveolen handelt. Später bilden die degenerierten Elemente des Surfactants eine mucoproteide Matrix, in der verschiedene Kalksalze ablagern.
Die Autoren berichten auch über 11 Fälle dieser Erkrankung. Sie nehmen an, daß bei einer initialen und unkomplizierten Pulmolithiasis subjective Beschwerden fehlen, weil die Mikrolithen frei in den Alveolen liegen und daher nicht geeignet sind, eine Störung im Sinne eines alveolo-capillären Blockes zu verursachen. Es wird auf einige röntgenologische Besonderheiten hingewiesen: verwaschene Herz- und Zwerchfellkonturen, ähnliche quantitative Charakteristik der röntgenmorphologischen Veränderungen bei dem familiären Auftreten und Befund der „Perlketten“ auf Übersichtsaufnahme beim fortgeschrittenen Stadium der Krankheit. Die Autoren schlagen eine neue röntgenologische Klassifikation der Pulmolithiais vor. Sie vertreten die Auffassung, daß die Krankheit pathognomonische Röntgenbilder anbietet, die erlauben, die Diagnose mit Sicherheit zu stellen.
