Abstract
Up to the 31st of December 1967 9100 (16,0/100000) cases of sarcoidosis were registered in the Public Health Offices of the Federal Republic of Germany. The real number of patients is estimated as high as 16000 to 30500 cases.
In 1968 in the three Public Health Offices of Stuttgart, Ludwigsburg, and Esslingen there were known 436 sarcoidosis patients among 1.13 million inhabitants (38.5/100000), 163 males (30.0/100000) and 273 females (46.6/100000), with a probable degree of known cases of 60±6.6%. The sex difference is statistically significant. Among women sarcoidosis is already more common than open tuberculosis. At the time of diagnosis men have been younger than women. The incidence of sarcoidosis in Stuttgart showed a rising tendency: 1962 5.2/100000, 1967 7.2/100000.
In 62% there existed symptoms, which gave rise to the diagnosis of sarcoidosis in only 30%: cough in 38%, respiratory dysfunction in 35%, and erythema nodosum in 32%. Erythema nodosum was more common among women than among men. 30% have been discovered by mass miniature X-Ray examinations. In 51% sarcoidosis has been diagnosed by the Public Health Office, in 27% by hospitals, and in 10% by practitioners.
Extrapulmonary manifestations were observed in one third of the cases: lymph nodes in 35% and skin lesions in 13%.
In 35% the diagnosis could be confirmed histologically. A positive result could be obtained in 61% of the biopsies. In all other cases the diagnosis was set up clinically and by follow-up, in 86% for at least 2 years. The tuberculin test gave a negative result in 50%.
At the time of diagnosis 58% of the patients were in the first stage of the disease; in 42% there were pulmonary infiltrations. The course of the disease proved to be rather benign with a regressive tendency in 56% and a stationary one in 24%; in 14% there was a progression of pulmonary infiltrations, in 7% the course was chronically relapsing.
A real influence of pregnancy (51 women) on the course of sarcoidosis could not be proved. From 1955–1968. 7 fatal cases of sarcoidosis came to our attention; that means a mortality of about 0.05/100000/year. 66% of the patients have been treated in hospitals for an average of 85 days (males) and 88 days (females) respectively. Ambulatory treatment was performed for 11%. 23% were not treated at all.
From the epidemiological point of view the age, sex and social distribution are not consistent with any causal relation between sarcoidosis and tuberculosis. Also the tuberculosis infection rate and exposition among sarcoidosis patients did not exceed the expectancy rate significantly.
It is expected that the morbidity rate of sarcoidosis will continue to increase.

In der BRD waren am 31. 12. 1967 9100 (16,0/100000) Sarkoidose-patienten bei den Gesundheitsämtern registriert. Der tatsächliche Bestand wird auf 16000 bis 30000 Fälle geschätzt.
Bei den 3 Gesundheitsämtern Stuttgart, Ludwigsburg und Esslingen waren 1968 436 Sarkoidosepatienten (38,5/100000 Einwohner) bekannt, darunter 163 Männer (30,0/100000) und 273 Frauen (46,6/100000) mit einem mutamaßlichen Erfassungsgrad von 60 ± 6,6%. Der Geschlechtsunterschied ist statistisch signifikant. Bei Frauen ist die Sarkoidose bereits häufiger als die ansteckende Lungentuberkulose. Männer waren bei Erstdiagnose jünger als Frauen. Inzidenzberechnungen für die Stadt Stuttgart ergaben eine steigende Tendenz (1962: 5,2/100000, 1967: 7,2/100000).
Beschwerden überhaupt kamen bei 62% der Patienten vor, führten aber nur bei 30% insgesamt zur Diagnose; dabei standen Husten (38%), Atembeschwerden (35%) und Erythema nodosum (32%) an der Spitze. Das Erythema nodosum fand sich signifikant häufiger bei Frauen als bei Männern. 30% wurden durch Röntgenreihenuntersuchungen entdeckt. In 51% wurde die Sarkoidose durch das Gesundheitsamt, in 27% durch Krankenanstalten und in 10% durch niedergelassene Ärzte diagnostiziert. Eine Beteiligung extrathorakaler Organe wurde in 1/3 der Fälle beobachtet; am häufigsten waren periphere Lymphknoten (35%) und die Haut (13%) befallen.
Bei 35% aller Patienten gelang die histologische Bestätigung der Diagnose, bei einem Anteil positiver Biopsien von 61%. Alle übrigen Fälle waren klinisch und durch Verlaufsbeobachtungen gesichert. Die Tuberkulinprobe fiel in 50% negativ aus.
Zum Zeitpunkt der Erstdiagnose befanden sich 58% der Patienten im Stadium I; bei den restlichen 42% war das Lungenparenchym erkennbar beteiligt. Mit regredientem Verlauf in 56% und stationärem Verhalten in 24% erwies sich die Sarkoidose als relativ gutartig; immerhin kam es in 14% zur Progression der Lungenveränderungen und in 7% zu Rezidiven.
Ein wesentlicher Einfluß einer Schwangerschaft (51 Frauen) auf den Verlauf ließ sich nicht nachweisen.
In 13 Jahren wurden 7 Sterbefälle bekannt; das entspricht einer Mortalität von etwa 0,05/100000/Jahr. 86% der Patienten konnten länger als 2 Jahre beobachtet werden. 66% der Patienten wurden wegen ihrer Erkrankung im Durchschnitt 85 Tage (Männer) bzw. 88 Tage (Frauen) behandelt. 11% wurden lediglich ambulant, 23% überhaupt nicht behandelt.
Epidemiologisch sprechen Alters- und Geschlechtsverteilung sowie soziale Schichtung gegen einen Zusammenhang zwischen Sarkoidose und Tuberkulose. Auch Tuberkulose-belastung und -exposition wurden nicht häufiger als zu erwarten angetroffen.
Ein weiterer Anstieg der Sarkoidosehäufigkeit ist wahrscheinlicher als das Gegenteil.
