Außerordentliche Überempfindlichkeit auf tuberkulöse Antigene nach BCG-Vakzination mit Leber-, Milz- und Lymphknotenschädigung

Chronic BCG mycobacteriosis?

Unusual hypersensitivity to tuberculous antigens after BCG vaccination, with liver, spleen, and lymph node damage

Zusammenfassung

Bei einem gegenwärtig achtzehnjährigen Jungen entwickelte sich im Säuglingsalter nach BCG-Vakzination eine chronische Erkrankung mit atypischer Überempfindlichkeit auf tuberkulöse Antigene. Im Verlauf der bisher andauernden Krankheit wurden epitheloid-riesenzellige granulomatöse Leber- und Milzentzündungen festgestellt. Es kam weiterhin zu stürmischen und sich wiederholenden Staphylokokken-Lymphadenitiden. Einige der betroffenen Lymphknoten verkalkten. Nach Tuberkulinproben kam es zu schweren Allgemein- und Lokalreaktionen.

Unseres Erachtens könnte es sich um eine chronisch verlaufende BCG-Mykobakteriose handeln. Der klinische Verlauf, die histologischen und serologischen Reaktionen zeugen gegen Sarkoidose wie auch gegen progredient septische Granulomatose (Dysphagozytose).
