Zusammenfassung
Minimal infizierte Meerschweinchen wurden für 2, 4 und 8 Wochen mit verschiedenen INH-Dosen prophylaktische behandelt. 25 und 50 mg/kg erwiesen sich nicht wirksamer als 10 mg/kg. Durch Erhöhung der Dosis ist keine Verkürzung der Behandlungszeit ohne Wirkungsverlust erzielt worden. Das gleiche gilt für das der sekundären Prophylaxe besser entsprechende Modell der chronischen Mäusetuberkulose. Trotzdem erscheint eine kontrollierte Prüfung kurzfristiger INH-Prophylaxe beim Menschen gerechtfertigt, weil unter gewissen epidemiologischen Verhältnissen selbst eine nur schwach wirksame einmalige INH-Massenprophylaxe unter Umständen eine stärkere Reduktion der Tuberkulose herbeiführen könnte als eine 20 Jahre lang durchgeführte BCG-Impfung aller Neugeborenen.
Summary
Minimally infected guinea pigs were treated prophylactically with different doses of INH for 2, 4 and 8 weeks. 25 mg/kg and 50 mg/kg daily were not more effective than 10 mg/kg. By rising the dose it was not possible to shorten the treatment period without loss of effectiveness. The same holds true for a model better adapted to the conditions of secondary prophylaxis: the chronic tuberculosis of the mouse. Nevertheless, a controlled trial of short INH medication in human beings seems justified because under certain epidemiological conditions even a weakly effective mass prophylaxis organized once might produce a better reduction of tuberculosis than BCG vaccination of all newborns for twenty years.
