Summary
Two characteristics of the ultrasonic image, that it is rebuilt and non figurative in its details, led us to establish specific relationships between anatomic structures and their ultrasonic images. The anatomic study was made on fresh, injected, frozen, livers, sectioned along the classical echographic planes. The echographic study was conducted in healthy patients, without any hepatic lesions. The results fall under three heads: analysis of the basic structures (portal vein, hepatic vein, and bile ducts) enables to establish perfect relationship between the anatomic and echographic features; the echo-anatomic study of the echographic planes pertaining to percutaneous examination of the liver provides a satisfactory correlation between the anatomy and the ultrasonogram; analysis of the hepatic vascular arrangement enables to define a sectorial systematization of liver structure. Segmental systematization of the right side of the liver is more difficult to define because of the virtual limits of the upper and lower segments of the right paramedian and lateral sectors. This study leads to better echographic understanding of the liver and to a tentative systematization essential to the localization of focal hepatic lesions.
Résumé
Le caractère reconstruit et non figuratif dans ses détails de l'image échographique nous a conduit à établir des corrélations précises entre les structures anatomiques et leurs images échographiques. L'étude anatomique porte sur des foies frais, injectés, congelés et coupés selon les plans de coupes échographiques classiques. L'étude échographique a été réalisée chez des sujets sains, indemnes de toute lésion hépatique. Les résultats se divisent en trois chapitres: l'analyse des structures élémentaires (veine porte, veine hépatique, canaux biliaires) permet d'établir une corrélation anatomo-échographique parfaite; l'étude écho-anatomique des plans de coupe spécifique à l'examen percutané du foie aboutit également à une bonne confrontation entre anatomie et échographie; l'analyse de la structure angiocanalaire du foie permet d'aboutir à une systématisation précise quant à la sectorisation hépatique. La systématisation segmentaire est plus difficile à définir au niveau du foie droit du fait des limites virtuelles des segments supérieurs et inférieurs des secteurs paramédian et latéral droits. Ce travail aboutit à une corrélation anatomo-échographique convenable tant au point de vue analytique que synthétique. Il débouche sur une meilleure connaissance du foie en échographie et une tentative de systématisation indispensable à la localisation des processus focaux intrahépatiques.
